兰州市城关区性病门诊男性就诊者艾滋病哨点监测结果分析

Analysis of results of sentinel surveillance of male patients in venereal disease clinic in Chengguan District of Lanzhou

目的监测甘肃省兰州市城关区性病门诊男性就诊者HIV、HCV、梅毒感染现状和艾滋病相关高危行为发生情况,为制定艾滋病防控措施提供科学依据。方法依据国家艾滋病哨点监测方案,对2011─2013年主动前往性病门诊就诊的1 200名男性就诊者进行问卷调查,并采集血样检测HIV、HCV和梅毒。结果 1 200名男性就诊者艾滋病防治知识总知晓率为86.08%,最近三个月与商业性女性性服务人员发生性行为的比例为22.33%,最近三个月与临时性伴发生性行为比例为22.83%,与同性发生过肛交性行为比例为2.75%;检测HIV抗体阳性4例(0.33%),梅毒抗体阳性59例(4.92%),HCV抗体阳性25例(2.08%)。结论性病门诊男性就诊者中,有较普遍的商业性异性性行为和临时性伴性行为,部分既往有性病史,同时发现HIV、HCV和梅毒感染新发病例;今后应加强对性病门诊男性就诊者安全性行为和安全套推广等行为干预,同时提供规范的性病诊疗和咨询检测服务,减少HIV和相关性病的蔓延。

Objective To monitor male patients infected with HIV, HCV, syphilis and their AIDS-related high-risk behavior,to provide scientific basis for formulating measures of AIDS prevention and control. Methods According to national schedule for AIDS sentinel surveillance, questionnaire survey was conducted among 1 200 male patients initiatively visited the clinic from 2011 to 2013, and blood samples were collected to detect HIV, HCV and syphilis. Results Total awareness rate of AIDS knowledge was 86.08% in the 1 200 male patients. The proportion was 22.33% for sexual behavior with female commercial sex workers in the latest 3 months, 22.83% for the sexual behavior with casually sexual partners in the latest 3months, and 2.75% for the anal sexual behavior with same-sex partners. There were detected positive antibody in 4 cases（0.33%） with HIV, 59 cases（4.92%） of antibody positive, and 25 cases with HCV（2.08%）. Conclusions The commercial heterosexual sex and casual sex behavior is common in the local male patients in the clinic. Some of them have history of venereal disease, and cases infected with HIV, HCV and syphilis are also found. Behavior intervention such as developing safe sexual behavior and extending condom use should be strengthened in the male patients in the clinic. Meanwhile, normative diagnosis and treatment of venereal disease and service of counseling and detection should be provided for them in the future.
