摘要
目的探讨2型糖尿病患者血清血管内皮钙粘蛋白(VE-cadherin,VE-Ca)水平与血糖、血脂及2型糖尿病早期颈动脉粥样硬化(CAS)的关系。方法将新诊断T2DM患者90例,根据有无CAS分为合并CAS组(A组)48例和无CAS组(B组)42例,设健康对照组(C组)35例。测量体重指数、血压,检测临床生化指标和血清VE-Ca水平。结果3组血清VE-Ca水平依次为:C组(2.79±1.68)ng/mL<B组(4.53±1.42)ng/mL<A组(5.16±1.43)ng/mL,3组比较,差异有统计学意义(P<0.05或P<0.01)。血清VE-Ca与FPG、HbA1c、TG、SBP、DBP显著正相关(r分别为0.237、0.734、0.319、0.229、0.349,P<0.01)。经Logistic回归分析显示,VE-Ca、DBP、FPG、TC是T2DM发生CAS的主要影响因素。结论血清VE-Ca在新诊断T2DM合并颈动脉斑块人群中升高,在新诊断T2DM合并CAS的发生中起重要作用。
Objective To study the correlation of concentration of serum VE-cadherin(VE-Ca) with serum glucose, blood, lipid and the carotid atherosclerosis(CAS) in the newly diagnosed T2DM patients. Methods The 90 patients with newly diagnosed T2DM were divided into diabetic CAS group (Group A, n = 48),and diabetic non-CAS group (Group B, n = 42). 35 healthy subjects were enrolled in control group. Age, body mass index(BMI),TC,TG,HDL,LDL,HbAlc,VE-Ca were calculated and compared. Results The VE-Ca levels in Group C were(2.79±1.68)ng/mL, (4.53±1.42) ng/mL in Group B and (5.16±1.43)ng/mL in Group A. There was remarkably statistical difference between T2DM patients and health control (P〈 0.01) and statistical difference between Group A and Group B(P〈0.05 ). Positive correlations between the serum VE-Ca and FPG, HbAlc,TG,SBP and DBP(r=0.237,0.734,0.319,0.229,0.349,P〈0.01)were found. Logistic regression analysis showed that VE-Ca, DBP, FPG, and TC were the main factors influencing macrovascular complications in T2DM.Gonclusion The level of serum VE-Ca in newly diagnosed T2DM patients with CAS were higher and played a very important role in the pathogenesis of T2DM combined with CAS.
