低剂量CT在肺结节筛查中的临床应用 被引量:43

摘要 目的:探讨胸部低剂量螺旋CT扫描在健康人群中进行肺结节筛查的价值。方法:将700例健康体检者随机平均分为两组,分别进行常规剂量肺CT扫描或低剂量肺CT扫描,对比其得到的图像质量、肺结节检出率和患者辐射剂量。结果:常规剂量组、低剂量组两组图像质量均能满足临床诊断要求。常规剂量组检出肺结节32例37个,病理证实早期肺癌2例;低剂量组检出肺结节29例32个,病理证实早期肺癌1例,二者在检出率上没有明显统计学差异。而辐射剂量方面,常规剂量组的剂量长度乘积、平均有效管球剂量、总管球剂量和放射容积剂量指数分别为(512.64±43.96)mGy·cm、(115±9.76)mAs、(3210±143.34)mAs和(10.53±2.63)mGy。低剂量组分别为(173±18.45)mGy·cm、(52±3.43)mAs、(1178±40.87)mAs和(5.73±1.73)mGy,两组相比P值均<0.05,差异均有统计学意义。结论:胸部低剂量螺旋CT扫描对肺结节的显示及检出与常规剂量相同,且辐射剂量更小,更适合健康群体中肺结节的筛查。 Purpose:To compare the value of low-dose chest CT scan with conventional dose scan in pulmonary nodules screening in healthy population.Methods:Lung CT scans of 700 healthy individuals were analyzed.The image quality,detection rate and radiation dose were compared between the conventional dose group and the low-dose group.Results:There was no significant difference of the image quality and the detection rate between the two groups in finding the pulmonary nodules.In the conventional group,the dose length product (DLP),eff.mAs,total mAs and CT dose Index volume (CTDI vol) were (512.64±43.96) mGy cm,(115±9.76) mAs,(3210±143.34) mAs and (10.53±2.63) mGy respectively,while those in the low-dose group were (512.64±43.96) mGy·cm,(115±9.76) mAs,(3210±143.34) mAs and (10.53±2.63) mGy.There was significant difference between them (P〈0.05).Conclusion:The low-dose CT scan was with no significant difference from the conventional dose scan in screening pulmonary nodules,but the radiation dose of low-dose CT scan was lower.The chest low-dose spiral CT scan is more suitable for early lung cancer screening in healthy population.
