PNAM对单侧完全唇腭裂的矫治效果评价

Evaluation of the Clinical Effect of Presurgical Nasoalveolar Molding in Infants with Unilateral Complete Cleft Lip and Palate

目的:评估术前鼻-牙槽塑型治疗(presurgical nasoalveolar molding,PNAM)对单侧完全性唇腭裂(unilateral complete cleft lip and palate,UCLP)患儿鼻唇部和牙槽突部的矫治效果。方法:实验组和对照组均由30例单侧完全唇腭裂患儿组成。实验组患儿在唇裂手术前接受PNAM治疗。比较实验组患儿PNAM治疗前后及实验组和对照组患儿唇裂术前的鼻翼宽度、患侧唇峰间距离、鼻小柱高度、牙槽突裂隙宽度。采用SPSS 13.0软件对数据进行统计学分析。结果:实验组患儿PNAM治疗后牙槽突裂宽度,患侧鼻翼宽度,唇裂宽度明显缩小,患侧鼻小柱高度明显提高,与治疗前及对照组相比具有显著性差异。结论:术前鼻牙槽塑型治疗能显著改善单侧完全性唇腭裂患儿鼻唇部外形,减小牙槽突裂隙宽度,从而减小初次手术难度,提高手术治疗效果,是序列治疗的重要组成部分。

Objective：To evaluate the clinical effect of presurgical nasoalveolar molding（PNAM）in infants with unilateral complete cleft lip and palate（UCLP）.Methods：The experimental group and the control group were consisted of 30 UCLP infants.The presurgical orthodontic treatments with PNAM were performed in the experimental group but not in the control group.We analyzed the quantity changes of lip-nose deformity in the patients before and after treatments with PNAM in the experimental group and the quantity changes of lip-nose deformity in the patients between the experimental group and the control group before cheiloplasty.The data were analyzed with SPSS 13.0software.Results：After PNAM treatments in the experimental group,the alar width,the distance between the two lip peak in the cleft side and the width of the alveolar cleft were decreased significantly.The length of the columella was increased significantly.Compared with the control group,the quantity changes of lip-nose before cheiloplasty were significant.Conclusion：PNAM treatments in UCLP can improve nasal and labial profile and decrease the width of alveolar cleft,and make it easy for the primal operation of cleft lip and palate.PNAM can be a useful adjunct for sequence therapy.
