MSCT对肾脏嗜酸细胞瘤的诊断价值

Diagnostic value of multi-slice spiral CT in renal oncocytoma

目的探讨肾脏嗜酸细胞瘤的CT表现,以提高对该病的认识和诊断水平。方法回顾分析13例经手术、病理证实的嗜酸细胞瘤的CT特征,并与病理结果进行对照。结果 13例患者均为单发,7例位于右肾,6例位于左肾,10例向肾实质、肾盂方向生长,3例向肾外生长。CT平扫肿瘤主要呈等密度,2例可见钙化,增强主要表现为病灶明显强化,各期低于肾皮质但高于周围肌组织,强化特征呈"速升缓降"型;5例皮质期可见"轮辐状"强化,4例病灶内见低密度星状瘢痕影,3例可见囊状低密度影,1例病灶周围见出血。结论肾脏嗜酸细胞瘤的CT表现具有一定特征性,但特异性不高,最后诊断常需依靠病理证实。

Objective To explore the computed tomography （CT ） manifestations of renal oncocytoma for improving the diagnostic accuracy and understading of the disease .Methods Imaging features of 13 patients with renal oncocytoma con-firmed by surgical pathology were analyzed retrospectively and compared with pathological findings .Results All patients had solitary lesion .7 of 13 cases were located in right kidney and 6 in left .10 tumors growed mainly to the renal parenchy-ma or renal pelvis while 3 to the cortex outside of kindney .On pre-contrast CT scanning ,the tumors were shown mainly isodense and calcification were observed in 2 cases .On multiple phase contrast scanning ,most RO appeared obvious en-chancement ,the density were lower than that of the cortex but higher than muscles ,which displayed an enhancing phe-nomenon of ‘fast in and slow out’ sign .5 tumors were shown spoke-wheel-like enchancement in the cortico-medullary phase .Central stellate star scars were found in 6 lesions and small cystic low density was observed in 3 ,hemorrhage was seen around 1 lesion .Conclusion The CT manifestations of renal oncocytoma have certain features ,but the specificity of these characteristics is still low ,and the final diagnosis should be relied on pathology .
