摘要 目的:探讨颈椎Modic改变的MRI表现与临床意义。方法回顾性分析116例颈椎Modic改变的MRI图像资料,记录Modic改变的类型、节段分布情况。结果116例患者中共计150个节段存在Modic改变,按照Modic分型,其中Ⅰ型23个节段,Ⅱ型110个节段,Ⅰ-Ⅱ混合型13个节段,Ⅲ型4个节段,发生率分别为15.3%、73.3%、8.7%、2.7%。按发生节段其中C5-6占57个节段,C6-7占41个节段,C4-5占29个节段,其余23个节段,分布情况与椎间盘突出好发部位相符。结论Modic改变是颈椎退行性病变中的一种常见表现,是引起颈痛的原因之一,并与椎间盘退变密切相关,MRI是首要检查方法。 Objective To discuss MRI findings of cervical vertebral Modic changes and their clinical value .Methods We retrospectively analysed MR imagings of cervical vertebral Modic changes in 116 cases and recorded types and distribtion of Modic changes .Results There were 150 segments with Modic changes including 23 segments in type Ⅰ ,110 segments in type Ⅱ ,13 segments in mixed type Ⅰ - Ⅱ and 4 segments in type Ⅲ according to Modic typing .The prevalence of typeⅠ ,type Ⅱ ,mixed type Ⅰ - Ⅱ and type Ⅲ were 15 .3% ,73 .3% ,8 .7% and 2 .7% ,respevtively .The distribution of Modic changes were 57 segments in C5-6 ,41 segments in C6-7 ,29 segments in C4-5 and 23 segments in the rest cervical seg-ments ,matching up with the common place to cervical vertebral disc protrution .Conclusion Modic changes ,which are cosely associated with disc degenertion ,are common presentation in cervical vertebral digenertive disease and one of causes to neck pain .M RI is the first choice to M odic changes .
