结直肠混合性腺神经内分泌癌2例临床病理分析 被引量：8

摘要 目的探讨结直肠混合性腺神经内分泌癌(mixed adenoneuroendocrine carcinoma,MANEC)的临床病理学特征、诊断及预后。方法回顾性分析2例结直肠MANEC的临床病理学和免疫表型特征。结果例1男性,72岁,直肠见一7 cm×6.5 cm×5 cm大小隆起型肿物,镜下见肿瘤组织由管状腺癌和大细胞癌混合构成;例2女性,58岁,左半结肠腔内见一4 cm×4 cm×1 cm大小溃疡型肿物,与肠系膜根部一10 cm×7 cm×5 cm大小质嫩肿物相沿续,镜下见肿瘤组织由管状腺癌、少量黏液腺癌和小细胞癌构成。免疫表型:2例腺癌区域中CEA、CK20、Villin均阳性,三者在神经内分泌癌区域均阴性,CD56、Syn在神经内分泌癌区域均阳性,在腺癌区域均阴性。结论结直肠MANEC属于高度恶性肿瘤,容易出现淋巴结或肝转移,肿瘤的发生机制和分级仍不明确,有待于进一步研究。
