非股骨短缩截骨全髋关节置换术治疗CrowⅣ型髋关节发育不良 被引量:3

摘要 目的:观察非股骨短缩截骨全髋关节置换术治疗CrowⅣ型髋关节发育不良的临床疗效与安全性.方法:2002年1月至2008年2月,采用非股骨短缩截骨全髋关节置换术治疗CrowⅣ型髋关节发育不良患者20例21髋,均为女性.年龄36 ~58岁,中位数42岁.单侧19例19髋,双侧1例2髋.Trendelenburg征均为阳性.术前均行双侧髋关节正侧位X线及CT检查,确诊为CrowⅣ型髋关节发育不良.所有患者均表现为髋关节疼痛、行走困难,均经非手术治疗无效.术前Harris评分25~60分,中位数45分.术后随访观察并发症发生、肢体短缩纠正及髋关节功能恢复情况.结果:所有患者均获得随访,随访时间1~6年,中位数4年.3例3髋术中发生骨折,其中2例2髋股骨小转子轻微骨折,行钢丝环扎固定后股骨假体稳定,骨折愈合良好;1例1髋股骨大转子不全骨折,行钢丝“8”字固定后股骨假体稳定,骨折愈合良好.l例出现坐骨神经刺激症状,1例出现股神经刺激症状,均于术后1个月自行恢复正常.均无感染、髋关节再脱位、假体松动及深静脉血栓形成等并发症发生.肢体短缩均得到满意纠正,基本达到肢体等长或短缩小于2 cm.患者无跛行或仅有轻度跛行.术后Harris评分79 ~96分,中位数85分.髋臼杯外展36°~60°,中位数41°.重建的髋关节旋转中心向内移位0~5 mm,中位数3 mm;向上移位0~5 mm,中位数2 mm.结论:采用非股骨短缩截骨全髋关节置换术治疗CrowⅣ型成人髋关节发育不良,能基本恢复肢体长度,纠正肢体短缩,有利于患肢功能的恢复,并发症少,值得临床推广应用.
