摘要
目的探讨单核细胞趋化蛋白1(MCP-1)、基质金属蛋白酶9(MMP-9)、金属蛋白酶特异性抑制剂1(TIMP-1)在川崎病(KD)患儿的表达及其与冠状动脉病变(CAL)的关系。方法 45例川崎病患儿分为有CAL(CAL组,15例)和无CAL(NCAL组,30例)两组,另外选取20例因呼吸道感染发热患儿(F组)和10例健康儿童(C组)为对照。采用ELISA检测血清MCP-1、MMP-9和TIMP-1水平,RT-PCR检测单核细胞中MCP-1、MMP-9和TIMP-1mRNA表达水平,心脏彩超观察CAL情况。结果CAL组和NCAL组急性期、缓解期血清MCP-1、MMP-9、TIMP-1、MMP-9/TIMP-1比值及外周血单核细胞中MCP-1、MMP-9、TIMP-1 mRNA表达水平均高于F、C组(P<0.01)。CAL组急性期血清MCP-1、MMP-9、TIMP-1、MMP-9/TIMP-1比值及MCP-1、MMP-9、TIMP-1mRNA表达水平均高于NCAL组(P<0.01或P<0.05),KD患儿血清MMP-9/TIMP-l比值与左冠状动脉内径/主动脉瓣环内径比值呈正相关(r=0.502,P<0.05)。结论联合检测MCP-1和MMP-9有助于预测和早期诊断KD患儿的CAL。
Objective To explore the expressions of monocyte chemotactic protein 1(MCP-1),matrix metalloproteinase 9(MMP-9),tissue inhibitor of metalloproteinases 1(TIMP-1)and their relationship with coronary artery lesions(CAL)in the children with Kawasaki disease(KD).Methods A total of 45 children with KD was divided into groups of CAL(KD with CAL,15cases)and NCAL(KD without CAL,30cases).Twenty children with fever due to respiratory tract infection(group F)and 10 healthy children(group C)were taken as the controls.Serum levels of MCP-1,MMP-9and TIMP-1were detected by ELISA and MCP-1,MMP-9and TIMP-1 mRNA in mononuclear cells of peripheral blood were detected by RT-PCR.CAL was observed by echocardiography.The ratio of MMP-9to TIMP-1 was calculated.Results Serum levels of MCP-1,MMP-9 and TIMP-1,and MMP-9/TIMP-1ratio and the expressions of MCP-1,MMP-9and TIMP-1 mRNA in mononuclear cells of peripheral blood were all significantly higher in groups of CAL and NCAL in acute and remission stages than those in groups of F and C(P〈0.01),which were higher in KD children in acute stage than those in remission stage(P〈0.01 or P〈0.05).The MMP-9/TIMP-1ratio was positively correlated to the ratio of diameter of the left coronary artery to inner diameter of the aortic valve ring(r=0.502,P〈0.05).Conclusion Combined detection of MCP-1and MMP-9is helpful in the prediction and early diagnosis of CAL in KD children.
