喉癌患者改良额侧位喉部分切除喉功能重建术后的生存质量分析

An analysis on quality of life in laryngeal carcinoma patients underwent modified frontolateral partial laryngectomy without tracheotomy

目的探讨改良额侧位喉部分喉功能重建切除术对喉癌患者生存质量的影响。方法42例喉癌患者分别接受改良额侧位喉部分切除术(A组,24例)和喉垂直部分切除术(B组,18例)。采用欧洲癌症调查治疗组织生存质量核心调查表(EORTC QLQ-C30)及头颈癌症生存质量特异性模块(EORTC QLQ-H&N35)对两组患者术后的生存质量进行评估。结果术后15d,A组在角色功能、情绪功能、社会功能、疲乏、疼痛、感觉问题、语言问题、社交进食、社会关系、咳嗽、胃管留置和体重下降方面得分均优于B组(P<0.05);术后3个月时两组生存质量评分渐渐趋于一致,至术后12个月时,A组仅社会关系1项得分优于B组(P<0.05)。结论与喉垂直部分切除术比较,改良额侧位喉部分切除喉功能重建术后喉癌患者近期生存质量明显提高。

Objective To evaluate the quality of life in the patients with laryngeal carcinoma underwent modified frontolateral partial laryngectomy.Methods The quality of life of 42 patients with laryngeal carcinoma after surgery was evaluated with EORTC QLQ-C30（European organization for research into the treatment of cancer quality of life questionnaire C30）and EORTC QLQ-HN35（neck special scale HN35）,who received modified frontolateral partial laryngectomy（group A,24cases）and vertical partial laryngectomy（group B,18cases）.Results The scores of role function,emotional function,social function,fatigue,pain,feeling,language,social eating,social relation,cough,stomach tube indwelling and loss of weight on the 15 th day after operation were better in group A than those in group B（P〈0.05）.The scores of the quality of life in two groups in the 3rd month after surgery gradually tended to be similar.The social relation score alone was better in group A than that in group B one year later（P〈0.05）.Conclusion Compared with vertical partial laryngectomy,modified frontolateral partial laryngectomy shows a short-term advantage in the improvement of quality of life after surgery in the patients with laryngeal carcinoma.
