糖皮质激素对早期ARDS患者的治疗效果 被引量：5

摘要 目的探讨糖皮质激素对早期急性呼吸窘迫综合征(ARDS)患者的治疗效果。方法44例ARDS患者随机均分为两组:B组给予常规治疗;A组加用氢化可的松100mg,每8小时1次,连用7d。A组根据促肾上腺皮质激素(ACTH)刺激试验分为对激素有反应组(A1组,11例)和无反应组(A2组,11例)。比较治疗第1-8天氧合指数(OI)、血乳酸(Lac)、气道峰压(PIP)、气道平台压(Pplat)和平均气道压(Pm)。结果治疗第5-8天,A组OI高于B组(P<0.05);治疗后Lac呈下降趋势,A组低于B组(P<0.05);两组患者PIP、Pplat和Pm在治疗后呈下降趋势,治疗第3-8天,A组明显低于B组(P<0.05)。治疗前,A1组和A2组的OI、PIP、Pplat、Pm和Lac值差异均无统计学意义(P>0.05)。治疗后第6-8天,A1组OI明显高于A2组(P<0.05);治疗后第3-8天,A1组Lac明显低于A2组(P<0.05);治疗第7-8天,A组PIP、Pplat和Pm明显低于A2组(P<0.05)。结论小剂量激素对早期ARDS患者,尤其肾上腺皮质功能正常患者,有明显治疗效果。
