显微镜联合鼻内窥镜治疗外伤性泪道损伤58例疗效分析

Endoscopic treatment of traumatic microscopy combined with nasal Analysis of 58 cases of lacrimal duct damage

目的 观察显微镜联合鼻内窥镜治疗外伤性泪道损伤的疗效.方法 我们对收治的58例（58眼）患者根据不同的泪道损伤情况,均在显微镜联合鼻内窥镜下行泪道断端吻合、泪道损伤修复.结果 56例患者术后均随访6月至1年,术后均无明显瘢痕,无泪小点和下睑外翻畸形.54例术后无泪溢,冲洗泪道通畅.2例拔管后出现泪溢症状,泪道冲洗提示泪小管断端瘢痕形成,先行高频电泪道浚通,再置管3月～6月后拔管,泪道通畅.2例失访.结论 显微镜联合鼻内窥镜治疗外伤性泪道损伤可取得满意的疗效,特别是对于复合性泪道损伤,眼鼻相关组织结构损伤,具有明显优势,值得推广.

Observation of microscope and curative effect of endoscopic treatment of traumatic lacrimal duct injury. Methods We treated 58 cases （58 eyes） of lacrimal duct injury patients according to different situation, both in the microscope combined with nasal endoscope of lacrimal duct anastomosis of lacrimal duct injury,repair. Results 56 patients were followed up for 1 years after June,there were no obvious scar,no puncta and lower eyelid ectropion deformity. In 54 cases no epiphora,irrigation of lacrimal passage. 2 cases occurred after extubation epiphora, irrigation of lacrimal passage prompt lacrimal canaliculus broken end of scar formation, the first high frequency electrolacryocystoptasty then catheter, March 6 months after extubatlon, lacrimal passage. 2 patients were lost to follow-up. Conclusion Microscopy combined with nasal endoscopic treatment of traumatic lacrimal duct injury can obtain satisfactory curative effect, especially for complex lacrimal duct damage, damage the eye nose related organization, has obvious advantages, is worthy of promotion.
