侯玉芬诊治下肢慢性溃疡的经验

Hou Yufen's Experiences in the Diagnosis and Treatment of Chronic Ulcer of Lower Limbs

侯玉芬教授是第四、五批全国名老中医药专家继承工作指导老师,擅长周围血管疾病的诊治,临证注重内治和外治结合,整体辨证和局部辨证结合,并且运用外治法有独特的经验。文章总结侯玉芬教授治疗下肢慢性溃疡的经验,重视疾病的病因诊断,针对发病原因采取不同治疗方法;内治以整体辨证论治为主,强调疾病的分期,早期以清热解毒为主,中期以清热利湿为主,后期以益气养血为主;外治以局部辨证为主,根据疮面的颜色、疮周的形态、分泌物的形状、气味等灵活选用溻渍、外敷、油膏纱布或软膏外敷等多种外治方法治疗;并将手术疗法、植皮疗法、加压疗法、功能锻炼等疗法综合运用,临床取得显著疗效。

Prof. Hou Yufen is the instructor for the inheritance work of national distinguished experts of Chinese medicine and is good at the diagnosis and treatment of peripheral vascular disease. The clinical treatment focuses on the integration of internal therapy and external therapy and the combination of holistic syndrome differentiation and local syndrome differentiation. The unique experience has been accumulated in the external therapy. Prof. Hou Yufen＇s experience was summarized in the paper on chronic ulcer of lower limbs. The disease is diagnosed by focusing on etiology and the different therapeutic methods are adopted in terms of causative factors. For the internal therapy, the disease is treated in stages according to holistic syndrome differentiation. The treatment focuses on clearing away heat and detoxification in the early stage, on clearing away heat and removing damp in the middle stage and on benefiting and nourishing qi in the late stage. For the external therapy, the different topical medicines are used flexibly according to the wound color, forms and secretions. The comprehensive therapy is applied additionally such as surgery, skin implantation, compression and functional exercise. The clinical efficacy is significant.
