花宝金运用气机升降理论治疗食管癌经验

Hua Baojin's Experiences in Esophagus Cancer with the Theory of Qi Ascending and Descending Activity

气机升降理论是指导临床诊治疾病的基本理论之一,其受到历代医家推崇并将其贯穿在诊疗和学术思想之中。花宝金教授善于运用经方治疗各种恶性肿瘤,并重视气机升降理论在肿瘤中的应用,认为气机升降失调是肿瘤发生的基本病理过程。花宝金教授认为食管癌发病的基本病理过程为情志因素、脾胃虚弱导致胃气上逆和脾气下陷,进而引起清浊之气郁滞,最终形成痰、瘀、毒等肿瘤病理产物而导致本病的发生。治则上结合食管"通降"的生理特性进行调治,治法上应调肝与调脾胃相互结合,以恢复气机升降的平衡;其临床代表方剂旋覆代赭泻心汤蕴含着以通降为主的气机升降理论思想,其在食管癌治疗中取得了较好的疗效。

Qi ascending and descending activity is one of the basic theories in the diagnosis and treatment of disease and has been highly praised and used in the treatment and academic thoughts. Prof. Hua Baojin is good at using classical formulas in treatment of various malignant tumors and attaches the great importance to qi ascending and descending activity theory. Prof. Hua believes that the basic pathological process of the occurrence of esophagus cancer is emotional disturbance and spleen and stomach deficiency that lead to upward reverse of stomach qi and sinking of spleen qi, as well as retention of turbid qi. Finally, the pathological products of tumor such as phlegm, stasis and toxin are formed. Regarding the treating principle, the disease is treated in terms of the＂ descending＂ physiological characters of esophagus. Concerning to the treating method, the liver, spleen and stomach are regulated coordinately to restore the balance of qi ascending and descending. The representative formula in clinic is xuanfu daizhe xiexin tang on the basis of the theory. This for- mula achieves the good efficacy on esophagus cancer.
