外治法预防痔术后伤口感染200例临床观察

Clinical Prevention of Postoperative Wound Injection in 200 Cases of Hemorrhoid Treated with External Therapy

目的观察中医外治法预防痔术后伤口感染的疗效。方法将200例痔术后患者随机分为治疗组和对照组,每组100例。治疗组患者术后伤口采用中药熏药、中药换药;对照组采用抗生素配合肛门熏洗、西医换药治疗。观察两组患者伤口有无感染症状及愈合时间。结果治疗组中内痔、外痔术后有效率与对照组比较,差异无统计学意义(P>0.05)。治疗组中混合痔切口在3个以下的有效率与对照组比较,差异无统计学意义(P>0.05)。治疗组中混合痔切口在3个及以上的有效率与对照组比较,差异有统计学意义(P<0.05)。两组的切口愈合时间比较差异无统计学意义(P>0.05)。结论中医外治法的综合应用,能够有效预防内痔、外痔及3个以下切口的混合痔术后伤口的感染,可以减少术后对抗生素的应用依赖。

Objective To observe the preventive effect on wound infection after hemorrhoid operation treated with external therapy of Chinese medicine. Methods Two hundred cases of postoperative hemorrhoid were randomized into a treatment group and a control group, 100 cases in each one. In the treatment group, Chinese medicine fumigation and dressing were adopted on the postoperative wound. In the control group, the antibiotics were applied together with anal fumigation and western medicine dressing. The infectious symptoms and healing time on the wound were observed in the patients of the two groups. Results The effective rate after internal hemorrhoid and external hemorrhoid operation in the treatment group was not different significantly as compared with the control group （ P 〉 0.05 ）. As compared with the control group, the difference in the effective rate was not significant on the less than 3 incisions of mixed hemorrhoid operation in the treatment group （ P 〉 0.05 ） ; but that on the over 3 incisions was significant （ P 〈 0.05 ）. The incision healing time was not different significantly in comparison of the two groups （ P 〉 0. 05 ）. Conclusion The comprehensive application of Chinese medicine external therapy prevents effectively from the wound infection on the less than 3 incisions after mixed hemorrhoid operation and reduces the dependence of postoperative antibiotics.
