中西医结合分期辨识高血压病探讨

Discussion of Hypertension Differentiated in Stages with Integrated Chinese and Western Medicine

高血压病是临床最常见的心血管疾病之一,现代医学对原发性高血压的诊断和分期分级非常明确。中医虽无此病名,但与"眩晕""头痛""肝风"等病的范畴十分密切,而中医对此病的分期分型至今尚无统一定论,其发生与气郁—血郁—痰湿—痰阻—血瘀—瘀血的病理过程关系十分密切,其"郁"在早期,"瘀"在"郁"后。从中西医结合角度分析认为其病机可归于早期气郁—血郁期和中、晚期血瘀—瘀血期。通过对此病进行中西医分期,可以从不同侧面提高对本病的认识,并进行中西医早期干预有助于高血压病的一级预防,同时通过中西医结合药物疗法对已出现的高血压病可以达到帮助性治疗的目的,并有助于二级预防以防止其严重并发症的发生。

Hypertension is one of the most common cardiovascular diseases in clinic. Modern medicine has very definite diagnosis and stages/phases of primary hypertension. TCM has no special term of this disease ,but classified it closely in the category of＂ dizziness＂,＂ headache＂ and＂ liver wind＂, etc. There is no unified determination on the stages,phases and types of the disease in TCM yet. TCM thinks the occurrence of disease is closely related to the pathological process as qi stagnation, blood stagnation, phlegm damp, blockage of damp, blood obstruction and blood stasis. Stagnation is predominated in the early stage and stasis/obstruction in the later. In view of integration of Chinese and western medicine and pathogenesis, hypertension of early stage refers to qi stagnation and blood stagnation stage, and that of middle and late stages refers to blood stagnation and blood stasis stage. By the recognition of stages in integrated Chinese and western medicine, the disease can be analyzed from different views and early intervention of integrated Chinese and western medicine benefits the first - grade prevention. Additionally, the medication of integrated Chinese and western med- icine contributes to the assistant treatment of hypertension and benefits the second - grade prevention so as to prevent from serious complications.
