uPA、Ki-67和VEGF在宫颈癌组织中的表达及相关性研究 被引量:19

摘要 目的探讨尿激酶型纤溶酶原激活物(uPA)、增殖细胞核抗原(Ki-67)和血管内皮生长因子(VEGF)在宫颈浸润癌组织中的表达及与临床病理参数的关系。方法采用免疫组织化学SP法,分别检测uPA、Ki-67及VEGF蛋白在94例宫颈浸润癌(ICC),83例宫颈上皮内瘤样病变(CIN)和23例正常宫颈上皮(NCE)中的表达,并分析其与宫颈癌临床病理学参数间的关系及三者间的相关性。结果免疫组化方法显示:从NCE到CIN再到ICC,uPA蛋白阳性表达率分别为8.7%、44.6%和71.3%;VEGF蛋白阳性表达率分别为17.4%、43.4%和76.6%;Ki-67蛋白阳性表达率分别为13.0%、34.9%和67.0%;差异均有统计学意义(P<0.01)。uPA和VEGF在ICC中的表达与盆腔淋巴结转移和临床分期有关(P<0.01);而与年龄和病理学类型、组织学分级无关(P>0.05);Ki-67在ICC中的阳性表达与临床分期有关(P<0.01),而与年龄、盆腔淋巴结转移、病理学类型及组织学分级无关(P>0.05)。在宫颈癌组织中,uPA、Ki-67及VEGF蛋白表达两两间均呈显著正相关(r=0.705,P<0.01;r=0.871,P<0.01;r=0.628,P<0.01)。结论 uPA、Ki-67及VEGF与宫颈癌的发生、发展密切相关,联合检测这些指标有助于判断宫颈癌的预后及干预治疗。 Objective To investigate the expressions of urokinase-type plasminogen activator(uPA),vascular endothelial growth factor(VEGF)and Ki-67 in early invasive carcinoma of the cervix and their relationship with the clinical pathological parameters.Methods Expressions of uPA,Ki-67 and VEGF were immunohistochemically detected in 94 cases of invasive cervix carcinoma(ICC),83 cases of cervical intraepithelial neoplasm(CIN)and 23 cases of normal cervical epithelia (NCE).The relationship between the expressions and the clinical pathological parameters of cervical carcinoma were analyzed statistically.Results In NCE,CIN and ICC tissues,the positive rate of uPA protein expression was 8.7%,44.6% and 71.3%,respectively,and that of VEGF protein 17.4%,43.4% and 76.6%,respectively,and that of Ki-67 protein 13.0%,34.9% and 67.0%,respectively(P〈0.01).The expressions of uPA and VEGF correlated with pelvic lymph node metastasis and clinical stage(P〈0.05)but not with patient's age,histological grades and pathological types(P〉0.05).The expression of Ki-67 correlated with clinical stage(P〈0.01)but not with patient's age,pelvic lymph node metastasis,pathological types and histological grades(P〉0.05).In ICC tissues,there was obviously positive correlation between uPA and VEGF expressions(r=0.871,P〈 0.01),between uPA and Ki-67 expressions(r =0.705,P 〈 0.01),and between VEGF and Ki-67 expressions (r =0.628,P 〈0.01).Conclusion The positive expressions of uPA,VEGF and Ki-67 in ICC may play an important role in the occurrence and development of ICC.Detection of these proteins combined helps to estimate the prognosis of ICC patients and provide interven tion treatments.
