Human papillomavirus infection: Trend report in 5 511 women

目的：了解门诊就诊的高危人群人乳头瘤病毒（ HPV）感染情况及分型特点，为该区宫颈癌的防治和HPV疫苗的研发提供参考。方法：回顾性分析2012年1月～2013年12月我院妇科门诊5511例女性宫颈细胞学标本的HPV基因分型检测结果。结果：①5511例受检者中HPV总感染率为17．13％，高危亚型组优势型别依次为16、58、52、33、18、31，低危亚型组依次为11、6、44。②单一、多重感染率分别为13．06％、4．06％，差异有统计学意义（χ^2＝285．012，P＝0．000）。③高危型、低危型、混合感染率分别为16．64％、0．96％、0．36％，差异均有统计学意义（χ2＝1667．737，P＝0．000）。④～44岁组、45～59岁组、60～岁组的感染率依次为17．98％、13．32％、32．93％，不同年龄段HPV感染率结果差异有统计学意义（χ^2＝47．102，P＝0．000）。结论：门诊高危人群HPV感染率为17．13％，常见型别为16、58、52；60～岁组为感染的高峰人群，应加强此类人群的检测，及早发现宫颈病变及宫颈癌。

Objective：To investigate the infection status and distribution of human papillomavirus （ HPV） genotypes in female outpatients in order to sup-ply evidences for the cervical cancer prevention and HPV vaccine development .Methods：HPV genotyping results were retrospectively summarized in 5 511 women undergone cytological screening of HPV at the outpatient of our hospital between January 2012 and December 2013.Results：①Positive HPV infec-tion was found in 17.13% of the 5 511 women,and the high-risk HPV was associated with genotype 16,58,52,33,18 and 31,and low-risk with 11,6 and 4;②The simple infection（13.06%;720/5 511） and the mixed infection（4.06%;224/5 511） were statistically different（χ^2 =285.012,P=0.000）;③The positive rate for high-risk,low-risk and mixed infection was 16.64%,0.96% and 0.36%,respectively,with statistical significance（χ^2 =1 667.737, P=0.000）;④The total HPV infection rate for patients under 44 years of age,between 45 and 59 years,or over 60 years was 17.98%,13.32% and 32.93%,respectively,and the infection rate was significant for different age groups （χ2 =47.102,P =0.000）.Conclusion：HPV infection rate was 17.12% in our outpatients,and HPV16,58 and 52 appear more prevalent.Besides,patients over 60 years of age are in highest risks of HPV infection,sug-gesting that this group of population shall be subjected to extensive HPV screening for early prevention of the cervical lesions or cancers .
