消癜汤对过敏性紫癜儿童肠道菌群的影响

Effect of Xiaodian Decoction on Intestinal Flora of Children with Henoch Schonlein Purpura

目的观察消癜汤治疗儿童过敏性紫癜（HSP）的疗效及对其肠道菌群的影响。方法 65例HSP患儿,试验组38例和对照组27例,对照组予西医常规治疗[口服氯雷他定片,体重〈30kg者每次5mg,体重≥30kg者每次10mg,每日1次;关节、消化道症状严重者予静滴甲强龙琥珀酸钠,1~2mg/（kg·d）,给药疗程〈1周],试验组在西医常规治疗基础上给予消癜汤,每日1剂。治疗2周后观察两组患儿临床疗效、临床症状改善及粪便标本中乳酸杆菌、双歧杆菌和大肠杆菌的含量变化。结果试验组临床疗效总有效率为92.11%,高于对照组的81.48%（P〈0.05）。在缓解腹痛的时间、改善尿隐血方面试验组优于对照组（P〈0.05）。试验组治疗后乳酸杆菌含量较治疗前显著增加,且乳酸杆菌含量显著高于对照组（P〈0.05）。结论消癜汤治疗儿童HSP疗效确切,其作用机制可能与调节肠道乳酸杆菌含量有关。

Objective To observe the therapeutic effect of Xiaodian Decoction on children with henoch schonlein purpura （HSP） and its effect on intestinal flora. Methods Totally 65 HSP children were randomized into the treat- ment group of 38 cases and control group of 27 cases. Both groups were given conventional western medicine treatment （oral administration of loratadine tablets, 5 mg/time for children under 30 kg and 10mg/time for children whose body weight equal to or above 30 kg once daily; intravenous infusion of methylprednisolone sodium succinate for children with severe symptoms of joints or digestive tract, 1 - 2 mg/ （ kg · d） ; The medication lasted less than one week）. The treatment group was added with Xiaodian Decoction one dose daily. The treatment lasted for two weeks. The clini- cal efficacy, symptom improvement and the content changes of lactobacillus, bifidobacterium and colibacillus in stool samples were observed. Results The total effective rate of the treatment group and control group were 92. 11% and 81.48% respectively, with a significant difference between groups （P 〈0.05）. The treatment group was superior to the control group in relieving abdominal pain and improving urine latent blood （ P 〈 0. 05）. The content of lactobacil- lus in the treatment group was significantly increased after treatment and it was significantly higher than that in the control group （P 〈 0.05 ）. Conclusion Xiaodian Decoction is effective for pediatric HSP and its possible mecha- nism may be relevant to regulating intestinal lactobaciltus content.
