环乳晕微切口治疗男性乳腺发育症的效果

The clinical efficacy of micro circum-areola incision on the recovery and patients' comfort of the male gynecomastia

目的探究环乳晕微切口治疗男性乳腺发育症的恢复效果及患者的舒适度。方法选取2009年5月～2013年5月河北省邯郸市中心医院就诊的男性乳腺发育症患者90例为研究对象,将其随机分为治疗组与对照组,每组45例。治疗组予以环乳晕微切口手术治疗,对照组予以传统开放式切除术。观察两组患者的手术时间、愈合时间、手术瘢痕、住院时间、舒适度评分以及主要影响舒适度的术后并发症如疼痛及水肿。结果治疗组患者的手术时间[（61.45±32.28）min]、愈合时间[（9.53±3.16）d]、手术瘢痕[（2.0±0.7）cm]及住院时间[（11.29±4.58）d]均明显少于对照组[（85.16±31.74）min、（12.10±3.05）d、（3.6±1.1）cm、（14.35±5.16）d],差异均有高度统计学意义（P〈0.01）;治疗组的术后疼痛8例、水肿3例,对照组疼痛21例、水肿10例,治疗组明显少于对照组,差异均有统计学意义（P〈0.05或P〈0.01）;治疗组患者的舒适度评分[（72.25±10.31）分]高于对照组[（58.84±12.28）分],差异有高度统计学意义（P〈0.01）。结论环乳晕微切口治疗男性乳腺发育症的恢复效果及患者舒适度明显优于传统开放式手术,建议临床进一步推广。

Objective To explore the clinical efficacy of micro circum-areola incision on the recovery effect and patient comfort of the male gynecomastia. Methods 90 cases of male gynecomastia in the Center Hospital Breast Surgery of Handan City from May 2009 to May 2013 were selected as the study objects. The patients were randomly divided into treatment group and control group, each group had 45 cases. The treatment group was given micro circum-areola incision, the control group was given traditional open surgery. The operation time, healing time, surgical scars, hospitalization and postoperative complications such as pain and edema, and patient comfort score of two groups were compared.Results The operation time of treatment group was（61.45±32.28） min, healing time was（9.53±3.16） d, surgical scars was（2.0±0.7） cm, hospitalization was（11.29±4.58） d, which were all lower than those of the control group [（85.16±31.74） min,（12.10±3.05） d,（3.6±1.1） cm,（14.35±5.16） d], and the differences were statistically significant（all P 〈0.01）. In treatment group, 8 cases of postoperative pain and edema in 3 cases, 21 cases with postoperative pain and 10 cases of edema in the control group, the treatment group was significantly less than those of the control group, the differences were statistically significant（P 〈0.05 or P 〈0.01）, comfort score of the treatment group was（72.25±10.31） points, that was higher than that of the control group [（58.84±12.28） points], the difference was statistically significant（P 〈0.01）. Conclusion Micro circum-areola incision can be better at improving the recovery effect and patients＇ comfort of the male gynecomastia compared with the conventional open surgery, recommend further promote in clinic.
