摘要
目的观察重组人酸性成纤维细胞生长因子(rh-aFGF)促进慢性溃疡性创面(静脉曲张性溃疡、压疮、糖尿病足溃疡)愈合作用及安全性。方法选择2012年8月~2013年8月于石家庄市中医医院、石家庄市第二医院、唐山协和医院三家医院收治的253例慢性溃疡患者,分为三组,试验组(90例)采用rh-aFGF冻干粉均匀外敷后喷雾治疗(3~4喷/cm2),对照组1(91例)则仅给予rh-aFGF喷雾治疗(3~4喷/cm2),对照组2(72例)采用湿润烧伤膏外敷后生理盐水喷雾治疗。观察三组的愈合时间、肉芽生长情况、瘢痕形成情况。结果试验组与对照组1和对照组2比较,患者创面渗出更少,肉芽生长状况更佳,差异均有统计学意义(P〈0.05)。试验组患者完全愈合时间、20 d累积愈合率、延迟愈合率均低于对照组1和对照组2,差异均有统计学意义(P〈0.05)。远期随访显示试验组瘢痕面积小于对照组1和对照组2,差异有统计学意义(P〈0.05)。结论 rh-aFGF促进愈合作用强于湿润烧伤膏,另外冻干粉直接外敷效果较喷雾的促进愈合效果更佳。
Objective To study the effect of recombinant human acidic fibroblast growth factor(rh-aFGF) on promoting chronic ulcerative wounds(varicose ulcer, pressure ulcer, pressuresorediabetic foot ulcer) healing. Methods From August 2012 to August 2013, in Hospital of Traditional Chinese Medicine of Shijiazhuang City, the Second Hospital of Shijiazhuang City, Union Hospital of Tangshan City, 253 patients with chronic ulcerative wounds were selected and divided into three groups, the experiment group(90 cases) were given rh-aFGF freeze-dried powder external application and aerosol therapy(3-4 sprinkle/cm2), the control group 1(91 cases) were given rh-aFGF aerosol therapy(3-4 sprinkle/cm2), the control group 2(72 cases) were given Shirunshaoshang Grease external application and normal saline aerosol therapy. The wound recovering time, granulation growth situation, cicatrization situation were observed. Results Compared with control group 1 and control group 2, wound exudation, granulation growth of experiment group were better, the differences were statistically significant(P 〈0.05). The complete healing time, 20 d cumulative healing rate and delayed union rate of experiment group were lower than those of control group 1 and control group 2, the differences were statistically significant(P 〈0.05). Long-term follow-up results showed the scar area of experiment group were smaller than those of control group 1 and control group 2, the differences were statistically significant(P 〈0.05).Conclusion Rh-aFGF is more effective than Shirunshaoshang Grease, and the rh-aFGF freeze-dried powder external application are superior to mist spray.
