阿维A联合凉血活血汤治疗斑块状银屑病的临床效果及安全性

Clinical efficacy and safety of Acitretin joint Liangxue Huoxue Soup in the treatment of plaque psoriasis

目的：探讨阿维A联合凉血活血汤治疗斑块状银屑病的临床效果及安全性。方法收集2011年6月-2013年6月来长庆油田职工医院就诊的140例诊断为斑块状银屑病的患者，参照随机数字表法将其分为对照组和联合组，各70例。对照组患者口服阿维A胶囊，外用他扎罗汀凝胶，联合组治疗在对照组的基础上给予凉血活血汤，观察两组树突状细胞（DC）的CD83、CD86表达、严重性指数（PASI）评分、治疗效果和不良反应、复发情况。结果治疗后1、3周，联合组PASI评分显著降低，差异有统计学意义（P〈0.05或P〈0.01），与对照组比较，差异有统计学意义（P〈0.05）。治疗后3周对照组PASI评分显著降低，差异有统计学意义（P〈0.05）。联合组有效率为97.1%，明显高于对照组（74.3%），差异有统计学意义（P〈0.05）。两组不良反应发生率比较，联合组明显低于对照组（1.43%比8.57%），差异有统计学意义（χ2=9.054，P〈0.05）。两组复发率比较差异有统计学意义（χ2=12.336，P〈0.05）。结论阿维A联合凉血活血汤治疗斑块状银屑病的临床效果明显，安全性较高，复发率较低，可在临床上广泛推广应用。

Objective To explore the clinical efficacy and safety of Acitretin joint Liangxue Huoxue Soup in the treat-ment of plaque psoriasis. Methods 140 cases diagnosed as plaque psoriatic patients were collected from June 2011 to June 2013 in Changqing Oil Field Worker’s Hospital, and divided into control group and combined group by the ran-dom number table method, with 70 cases in each group. Patients in the control group were given Acitretin Capsule by oral, Tazarotene Gel, patients in the combined group were given Liangxue Huoxue Soup on the basis of the control group. Dendritic cells （DC） of CD83, CD86 expression, severity index （PASI） score, therapeutic effect and adverse reac-tion, recurrence of two groups were observed. Results 1, 3 weeks after treatment, the PASI score of combined group de-creased significantly, the differences were statistically significant （P〈 0.05 or P〈0.01）, compared with control group, the difference was statistically significant （P〈0.05）. 3 weeks after treatment, PASI score in the control group decreased significantly, the difference was statistically significant （P〈 0.05）. Effective rate of 97.1% in the combined group was obviously higher than that of control group （74.3%）, the difference was statistically significant （P〈0.05）. The incidence of adverse reactions in the combined group was significantly lower than the control group, there was a statistically sig-nificant （1.43%v s 8.57%,χ2=9.054, P〈0.05）. The recurrence of similar between the two groups had a statistically sig-nificant difference （χ2=12.336, P〈 0.05）. Conclusion The linical efficacy of Acitretin joint Liangxue Huoxue Soup in the treatment of plaque psoriasis is obvious, and safety is high, the recurrence rate is low, which can be widely applied clinically.
