化疗对老年恶性肿瘤患者医院感染的影响

The influnce of chemotherapy on hospital infection of elderly patients with malignant tumor

目的：探讨化疗对老年恶性肿瘤患者医院感染的影响。方法前瞻性调查2009年4月～2014年1月在本院行化疗的250例老年恶性肿瘤患者，了解其医院感染的发生情况，并对医院感染率、感染部位、易感染肿瘤及医院感染的相关危险因素进行分析。结果250例化疗的老年恶性肿瘤患者中，共92例患者发生医院感染，医院感染的发生率为36.8%，其中呼吸系统是最易发生医院感染的部位，以肺癌的发生率最高，其中行联合化疗方案、采用静脉化疗及化疗开始至结束7d内是发生医院感染的高危因素，且化疗方案、化疗途径及感染时限均是造成老年恶性肿瘤患者医院感染的独立危险因素。结论老年恶性肿瘤化疗患者易发生医院感染，因此应加强临床护理，降低感染发生率，从而更好地提高临床疗效。

Objective To study the influnce of chemotherapy on hospital infection of elderly patients with malignant tumor. Methods 250 elderly patients with malignant tumor underwent chemotherapy from April 2009 to January 2014 were prospectively investigated.The occurrence of hospital infection was understood.The hospital infection rate,infection site,related risk factors of susceptible tumor and hospital infection were analysed. Results In 250 elderly patients with malignant tumor underwent chemotherapy,92 patients with hospital infection,hospital infection rate was 36.8%.The res-piratory system was the most easy to have the hospital infection,with the highest incidence rate of lung cancer.The com-bination chemotherapy,the use of intravenous chemotherapy and the start to the end of the risk factor of hospital infection occurred within 7 days chemotherapy were the risk factors of hospital infection.Chemotherapy regimen,chemotherapy pathway,infection time was independent risk factor for elderly patients with malignant tumor causing infection respective-ly. Conclusion Elderly patients with malignant tumor underwent chemotherapy prone to infection,and therefore should enhance clinical nursing care,reduce the incidence rate of infection in order to better improve the clinical efficacy.
