不同术式联合矫正老年性白内障患者术前散光的可行性研究

Feasibility study of different surgical combined with correction for senile cataract patients with preoperative astigmatism

目的：探讨不同术式联合矫正老年性白内障患者术前散光的效果和可行性。方法选取2013年1月～2014年6月本院收治的102例（102眼）老年性白内障散光患者作为研究对象,随机分为对照组（51眼）和观察组（51眼）,对照组行球面人工晶状体联合角膜缘松解术,观察组行超声乳化白内障摘除联合散光型人工晶状体（Toric IOL）植入术,随访3个月,比较两组裸眼视力、最佳矫正视力、柱镜度数和散光矫正率的差异。结果两组的最佳矫正视力比较,差异无统计学意义（P＞0.05）；观察组的裸眼视力、柱镜度数显著优于对照组,差异有统计学意义（P＜0.05）；观察组的散光矫正率为94.37%,显著高于对照组的63.38%,差异有统计学意义（P＜0.05）。结论 Toric-IOL和角膜缘松解术两种术式均是矫正老年性白内障患者术前散光的有效方法,Toric-IOL疗效更稳定,可保证术后最佳裸眼视力和视觉质量。

Objective To explore the clinical effect and feasibility of different surgical combined with correction for se-nile cataract patients with preoperative astigmatism. Methods 102 cases （102 eyes） of senile cataract patients with pre-operative astigmatism in our hospital from January 2013 to May 2014 were randomly divided into control group（51 eyes） and observation group（51 eyes）.The control group were treated with intraocular lens combined with corneal limbal lysis. The observation group were treated with phacoemulsification cataraction combined with Toric IOL.All patients were fol-lowed up for 3 months.The uncorrected visual acuity,best corrected visual acuity,columns diopters and astigmatism cor-rection rate of two groups were compared. Results The best corrected visual acuity of two groups had no significant dif-ference （P〉0.05）.The uncorrected visual acuity and columns diopters of observation group were significantly better than that of control group （P〈0.05）.The astigmatism correction rate of observation group was 94.37%,which was significantly higher than 63.38%of control group（P〈0.05）. Conclusion Toric IOL and corneal limbal lysis are both effective surgical methods to correct senile cataract with properative astigmatism.Toric IOL is more stable,it can ensure the best postoper-ative visual acuity and visual quality.
