年轻恶性及交界性卵巢肿瘤患者保留生育功能47例临床分析

Analysis of 47 cases of malignant and borderline ovarian tumors after fertility-preserving treatment

目的探讨年龄40岁以下恶性及交界性卵巢肿瘤患者保留生育功能的生存现状及治疗后月经、妊娠情况。方法回顾性分析2001年至2012年新泰市人民医院收治的年龄在40岁以下,接受保留生育功能治疗的恶性及交界性卵巢肿瘤患者的临床资料,并随访其生存及妊娠情况。结果 47例患者中恶性卵巢肿瘤无复发,死亡2例,交界性卵巢肿瘤复发5例,无死亡。43例有生育计划的患者中,20例成功妊娠,病理类型对妊娠无影响,未化疗组妊娠率明显高于化疗组。结论 40岁以下的年轻卵巢恶性肿瘤患者行保留生育功能的治疗后有良好的妊娠结局。子宫和对侧卵巢正常的恶性生殖细胞肿瘤患者均可行保留生育功能治疗;卵巢肿瘤剔除术的妊娠结局优于单侧附件切除术,化疗对妊娠结局有影响。建议对40岁以下妇女应定期进行妇科检查。

Objective：To explore the survival of the status quo and menstruation,pregnancy after treatment of malignant and borderline ovarian tumor in young women under 40 years old. Methods：A retrospective analysis from 2001 to 2012 in the People Hospital of Xintai and follow up their survival and pregnancy in young women under 40 years old who accept fer-tility preserving treatment of malignant and borderline ovarian tumor patients. Results：47 cases of malignant ovarian tumor patients without recurrence,2 cases died,5 cases of borderline ovarian tumor recurrence,no death. In the 43 patients de-sired to childbearing,20 cases of successful pregnancy,pathological type had no effect on pregnancy,the pregnancy rate was significantly higher than that of non chemotherapy group and chemotherapy group. Conclusion：Under the age of 40 young patients with malignant ovarian tumors for preserving fertility treatment after the good outcome of pregnancy. On the side of uterus and ovarian malignant germ cell tumor patients with normal are feasible fertility preserving treatment;ovarian tumor resection pregnancy outcome was better than that of unilateral accessory resection,chemotherapy has effect on preg-nancy outcome. Advice for women under 40 years old should be regular gynecological examination.
