高频电刀在环状混合痔外剥内扎术中的应用效果

Efficacy of High-frequency Electrotome Applied in External Dissection ＆ Internal Ligation for Circumferen- tial Mixed Hemorrhoids

为探讨高频电刀在环状混合痔外剥内扎术中的应用效果，将70例环状混合痔患者随机分为治疗组和对照组，各35例，治疗组在外剥内扎手术过程中使用高频电刀剥离外痔及止血，对照组在外剥内扎手术过程中使用剪刀剥离外痔，有活动性出血时使用慕丝线结扎止血。对比两组患者术中出血量及术后创缘水肿情况。结果显示，治疗组术中出血量明显少于对照组，P〈0．05；两组术后第、7、10、20天创缘水肿评分比较差异均无统计学意义，P〉0．05。结果表明，在环状混合痔外剥内扎术中应用高频电刀剥离外痔及止血，可有效减少术中出血量，将手术的出血危险系数降到最低，而且并未增加术后创缘水肿的风险，值得临床推广应用。

This study was to investigate the efficacy of high-frequency eleetrotome applied in external dis- section ＆ internal ligation for circumferential mixed hemorrhoids. The 70 patients with this hemorrhoids were randomly divided into treatment group and control group, n =35 for each group;during external dis- section ＆ internal ligation in treatment group high-frequency electrotome was used to dissect external hemorrhoids and to stop bleeding, while in control group scissors were used to dissect external hemor rhoids,and when active bleeding performing ligation hemostasis by using Mu silk thread;then the intraop- erative bleeding volume and postoperative status on wound-edge edema were compared between both groups.As results,on the 7th,10th and 20th day after operation in the score on wound-edge edema there was no statistical difference between both groups（ P〉0.05）, but the intraoperative bleeding volume of treatment group was significantly less than that of control group（ P〈0.05）.Results show that during ex- ternal dissection ＆ internal ligation for circumferential mixed hemorrhoids using high-frequency elect- rotome to dissect external hemorrhoids and to stop bleeding can effectively reduce intraoperative bleeding volume so that making the danger coefficient of operation bleeding decline to minimum rather than increase the risk of postoperative wound-edge edema, therefore using high-frequency electrotome is worthy to be popularized to use clinically.
