摘要
目的探讨基于细胞体外培养的干扰素释放试验对结核性胸膜炎辅助诊断的价值。方法采集结核性胸膜炎患者33例、非结核性胸膜炎组36例患者静脉血,应用结核分枝杆菌γ-扰素体外释放(TB-IGRA)诊断试剂检测静脉血细胞体外刺激培养后干扰素释放水平,同时抽取胸水常规检测腺苷脱氨酶(ADA),并与该常规方法进行比较。结果以全血细胞为检测样本体外干扰素释放试验对结核性胸膜炎检测的灵敏度93.9%,特异性88.9%,准确度91.3%;经比较干扰素释放试验灵敏度高于常规ADA检测方法(75.8%),差异有统计学意义(P<0.05)。结论基于细胞体外培养的干扰素释放试验检测方便、快捷,有助于提高对结核性胸膜炎的诊断效率。
Objective To investigate the value of auxiliary diagnosis to tuberculosis pleurisy based on the releasing assays of interferon cultured in vitro. Methods Venous blood was selected from 33 patients with tuberculous pleurisy and 36 patients with non tuberculous pleurisy, we detected the interferon releasing level of haemocytes stimulated and cultured in vitro by the application of mycobacterium tuberculosis gamma interferon (TB - IGRA) releasing detection agentia in vitro, meanwhile, extracted pleural effusion for routine detection of adenosine deaminase (ADA), and compared it with the conventional method. Results The sensitivity, specificity and accuracy of detecting tuberculous pleurisy is respectively 93.9% , 88.9% , and 91.3% according to interferon release assay in vitro with whole blood cells as detection samples; through the comparison, interferon release test sensitivity is higher than that of the conventional ADA detection method (75.8%), and the difference was statistically significant (P 〈 0.05). Conclusion The TB - IGRA is a convenient, quick tool. It might be improve the diagnostic efficiency of tuberculous pleurisy.
