N末端B型尿钠肽原联合肌钙蛋白Ⅰ对全身性感染患者心功能及预后评价 被引量:1

摘要 目的观察血浆N末端B型尿钠肽原(NT-proBNP)联合肌钙蛋白I(CTnI)水平对脓毒症患者心功能不全的预后价值。方法对入选的全身性感染患者动态检测血浆NT-proBNP及CTnI水平,并根据心脏超声结果分为心功能损害组(EF<50%)、心功能正常组(EF≥50%),及30天预后分为死亡组及存活组,比较两组间NT-proBNP及CTnI水平的不同,及其与APECHE-Ⅱ评分的相关性,计算ROC曲线下面积。结果心功能损害组NT-proBNP及CTnI较心功能正常者明显增高,死亡组患者两者血浆表达水平较存活组明显增高,并与APACHEⅡ具有很好的相关性,NT-proBNP及CTnI对死亡组预测均具有很好的敏感性及特异性(92.42%,80.03%;84.62%,75.53%)。结论 NT-proBNP及CTnI水平对全身性感染的预后有较高的诊断价值。
