纳米炭混悬注射液在宫颈癌手术治疗过程中示踪前哨淋巴结的临床应用 被引量:13

Clinical application of carbon nanoparticles suspension injection in sentinel lymph node tracing in the course of surgical treatment of cervical cancer

摘要 目的:对比不同淋巴示踪剂对宫颈癌患者经腹手术及腹腔镜手术中前哨淋巴结的检出效果。方法:随机选取100例在新疆医科大学附属肿瘤医院接受治疗的早期宫颈癌(Ⅰa-Ⅱa)患者并平均分为两组,实验组患者使用纳米炭混悬注射液示踪前哨淋巴结,对照组患者采取亚甲蓝与核素示踪前哨淋巴结,比较两组患者前哨淋巴结示踪剂的显影时间、检出率及假阴性率。结果:实验组的淋巴结示踪剂检出率为100.00%,假阴性率为0.00%;对照组的淋巴结示踪剂检出率为70.00%,假阴性率为30.00%,两组比较,差异有统计学意义(P〈0.05)。清扫的盆腔淋巴结主要分布在闭孔淋巴引流区,而在前哨淋巴结中闭孔淋巴结引流区的数目最多。结论:纳米炭混悬注射液可以很好地示踪宫颈癌术中淋巴结,避免不必要的淋巴结清扫,可降低对患者的损伤,为患者的预后提供保障。 Objective : To compare the detection effects of different lymph tracers for sentinel lymph node in the course of transab- dominal surgery and laparoscopic surgery of cervical cancer. Methods: A total of 100 patients with early cervical cancer ( stage Ⅰa-Ⅱa) treated in the hospital were randomly selected and divided into two groups; 50 patients in experimental group were treated with carbon nanoparticles suspension injection for sentinel lymph node tracing, while 50 patients in control group were treated with methylene blue and radionuelide for sentinel lymph node tracing. The developing time, detection rates and false negative rates in the two groups were compared. Restilts: The detection rate and false negative rate in experimental group were 100. 00% and 0. 00%, respectively; the detection rate and false negative rate in control group were 70.00% and 30.00%, respectively, there were statistically significant differences between the two groups (P 〈 0. 05 ) . The pelvic lymph nodes mainly located in obturator lymph drainage area, the number of sentinel lymph nodes in obturator lymph drainage area was the highest. Conclusion: Carbon nanoparticles suspension injection can detect lymph nodes during cervical cancer surgery, avoid unnecessary lymphadenectomy, reduce injury to the patients and provide protection for prognosis of the patients.
