摘要
目的:探讨高尔基体蛋白73(GP73)和甲胎蛋白异质体3(AFP-L3)联合检测在原发性肝癌诊断中的临床价值。方法:采用免疫组化法和酶联免疫法检测原发性肝癌组(PHC组,n=45例)、肝硬化组(LC组,n=35例)和病毒性肝炎组(VH组,n=36例)患者肝组织和血清GP73、AFP-L3和AFP的表达水平,选择同期40例门诊正常体检者作为对照组。观察各组肝组织和血清GP73、AFP-L3和AFP的表达水平,并比较GP73、AFP-L3、AFP单一检测和GP73联合AFP-L3检测原发性肝癌的阳性率。结果:PHC组肝组织和血清GP73、AFP-L3和AFP表达水平明显高于LC组和VH组(P<0.05);PHC组AFP-L3和GP73单项检测阳性率(分别为68.9%、77.8%)均明显高于AFP检测阳性率(62.2%)(P<0.05),两指标联合血清检测可提高阳性率(86.7%)(P<0.05)。结论:GP73和AFP-L3联合血清检测可提高原发性肝癌诊断阳性率,对原发性肝癌的早期诊断具有重要的临床意义。
Objective: To investigate the clinical value of Golgi protein 73 ( GP73 ) combined detection with alpha fetoprotein heterogeneity 3 ( AFP- L3 ) in the diagnosis of primary hepatic carcinoma. Methods: The expression levels of GP73, AFP- L3 and AFP in liver tissue and serum of primary hepatic carcinoma ( group PHC, n = 45 ), liver cirrhosis (group LC, n = 35) and viral hepatitis (group VH, n = 36) patients were detected by immunohistochemical and enzyme immunoassay. At the same period, 40 cases of normal examination were selected as control group. The expression levels of GP73, AFP-L3 and AFP in liver tissue and serum of all groups were observed, and compared the positive rate of GP73, AFP-L3, AFP single detection and GP73 combined detection with AFP-L3 to primary hepatic carcinoma. Results: The expression levels of GP73, AFP-L3 and AFP in the liver tissue and serum of PHC group were significantly higher than those of LC group and VH group ( P 〈 0. 05 ). The positive rate of AFP-L3 and GP73 in single serum detection (68.9%, 77.8% ) was obviously higher than that of AFP(62.2% ) in PHC group ( P 〈 0.05 ), but lower than that of AFP- L3 combined serum detection with GP73 ( 86.7 % ) ( P 〈 0. 05 ). Conclusion: The combined serum detection of GP73 and AFP- L3 can improve the positive rate of primary hepatic carcinoma. It has an important clinical significance on the early diagnosis of primary hepatocellular carcinoma.
