腹腔镜下微波固化治疗肝血管瘤的临床体会

Clinical experience in laparoscopic microwave coagulation for hepatic hemangioma

目的：探讨腹腔镜下微波固化治疗肝血管瘤的效果。方法对41例肝血管瘤患者采用腹腔镜下微波固化治疗。根据血管瘤大小，给予单点单次或多点单次微波固化。观察术后实验室检查结果、并发症发生情况及影像学检查判断治疗效果。结果所有患者均顺利完成微波固化治疗。术后4例患者出现转氨酶升高，2例患者出现发热。无出血、胆漏等并发症。随访4个月，完全损毁率为93．8％，部分损毁率为6．2％，所有患者均得到缓解。结论腹腔镜下微波固化是治疗肝血管瘤又一安全有效的微创治疗手段。

Objective To investigate the curative effects of laparoscopic microwave coagulation on hepatic hemangioma.Methods A total of 41 cases with hepatic hemangioma underwent laparoscopic microwave coagulation therapy.Based on the diameters of hemangioma,single-or multi-site microwave co-agulation was performed.The data of laboratory tests,postoperative complications and imaging examinations were analyzed to estimate treatment effects.Results The therapy was successfully completed in all pa-tients.The levels of transaminase increased in 4 patients,and there were 2 patients with fever.No patients had postoperative hemorrhage or bile leakage.During the follow-up of 4 months,remission were observed in all patients.The complete response was 93.8%（61/65）and partial response was 6.2%（4/65）.Conclu-sions Laparoscopic microwave coagulation is a safe,effective and minimally invasive therapy for hepatic hemangioma.
