心肌梗死后心脏性猝死一级预防心律转复除颤器植入最佳时机——争议与思考 被引量：1

Optimal timing of implantable cardioverter defibrillator for primary prevention implantation after myocardial infarction： controversy and thinking

摘要 2013年的指南[1]对于心肌梗死后心脏性猝死（SCD）-级预防植入心律转复除颤器（ICD）强调了2点：左心室射血分数（LVEF）≤0.30～0.35、急性心肌梗死40d以后.然而,心肌梗死40d后究竟哪个时期植入ICD效益最佳,目前尚缺乏足够具有说服力的资料.围绕心肌梗死后SCD一级预防植入ICD最佳时机和ICD对心肌梗死后降低心律失常性死亡率和增加非心律失常性死亡率的双重作用的可能机制引发了广泛关注与讨论.有研究观察到不适当ICD电击和不适时间植入ICD都可能显著降低患者的生活质量和增加死亡率[2-3].本文就近年有关心肌梗死后SCD一级预防植入ICD最佳时机研究资料和观点进行梳理和讨论,希望临床医师在指南的基础上更好地把握适应证.
