预先血常规和CRP检验在儿童门急诊流程优化中的价值

The value of BR test and CRP test in advance in children's outpatient and emergency process optimization

目的诊前做血常规检查,缩短发热患儿就诊时间,减少医疗投诉。方法我院门诊部增设了诊前服务站,分诊护士为需要等待就诊的发热患儿提前开具血常规化验单。结果 2013年患儿人均就诊时间较2012年缩短了29分钟。由化验单引发的医疗投诉由2012年的167例下降到2013年的26例。结论在进行资质考核的基础上给予分诊护士一定的授权[1],使分诊护士具有初步干预和开化验单的权利,可以较大程度地缩短发热患儿就诊时间,提升患者满意度,促进门诊医疗资源合理分配,值得国内各儿童医院借鉴与推广。

Objective Preclinical routine blood test can shorten the visiting time for children with fever and decrease medical complaints. Methods Preclinical service station was set up in our outpatient department and triage nurse could prescribe routine blood test for children with fever during the waiting time. Results The average visiting time was 29 minutes shorter in 2013 compared to that in 2012. Besides, the medical complaints caused by laboratory reports decreased to 26 cases compared to 167 cases in 2012. Conclusion Based on the qualification assessment, triage nurses were authorized to give basic intervention and prescribe laboratory examination, which could obviously shorten the visiting time for children with fever, improve the patients satisfactory, and promote reasonable distribution of out-patient medical resources. This is worthy of reference and promotion for the children＇s hospitals in China.
