双锁定钢板内固定治疗肱骨远端“C”型骨折的效果观察

Clinical observation on double-plate internal fixation for C-shaped fraction of distal humerus

目的观察双锁定钢板内固定对肱骨远端"C"型骨折的治疗效果。方法 2011年5月—2013年1月收治的96例肱骨远端"C"型骨折患者作为研究对象,按随机数字表法分为研究组和对照组。研究组手术治疗时给予双锁定钢板内固定,对照组手术治疗时给予"Y"型钢板内固定。观察两组患者术后的住院时间、骨折愈合时间。患者出院后随访12个月并记录肘关节功能的恢复情况及术后远期并发症发生情况。计量资料比较采用t检验,计数资料比较采用χ2检验,P<0.05为差异有统计学意义。结果研究组术后住院时间为(12.13±1.21)d,对照组(14.02±1.15)d,比较差异有统计学意义(P<0.05)。研究组肘关节功能恢复优良率85.42%,对照组优良率66.67%,比较差异有统计学意义(P<0.05)。研究组远期并发症发生率8.33%,对照组远期并发症发生率22.92%。比较差异有统计学意义(P<0.05)。结论在治疗肱骨远端"C"型骨折时,双锁定钢板内固定与"Y"型钢板内固定相比具有术后住院时间短、肘关节功能恢复优良率高和远期并发症少的优势,值得在临床上推广应用。

Objective To study the efficacy of double - plate internal fixation in the treatment of C - shaped fraction of distal humerus. Methods Ninety - six cases of C - shaped fraction of distal humerus were selected from May 2011 to January 2013 in our hospital. All cases were randomly divided into research group and control group. Research group were under- went the internal fixation of double - plate, while control group received the internal fixation of Y - shaped plate. Both groups were. observed and compared for hospital stay and fracture healing time. All patients were followed up for 12 months and recorded for the recovery of elbow function and long - term and postoperative complications. Measurement data were processed by the t test, count data by x^2 test, the result of P 〈 0. 05 was considered statistically significant. Resulls The postoperative stay in hospital was （ 12.13 ±1.21 ）d in research group and （ 14.02 ± 1.15）d in the controls, which showed significant difference（ P 〈 0. 05）. The excellent of the recovery of elbow function was 85.42% in research group and 66. 67% in the controls, which showed significant difference（ P 〈 0.05 ）. The incidence of longterm complications was 8. 33% in research group and 22.92% in the controls, which showed significant difference（ P 〈0.05 ）. Conclusions For C - shaped fraction of distal humerus, the internal fixation with double - plate is superior to the internal fixation with Y - shaped plate, for it is of short hospital stay, higher excellent rate of recovery of elbow function and less long - term postoperative complications. Therefore, it is worthy of clinical application.
