胃肠积热证内涵之研究进展

A Review to Explore the Development of Gastrointestinal Heat Retention Syndrome

[目的]明确胃肠积热证的内涵,促进胃肠积热证相关性疾病的诊疗和科学研究。[方法]查阅CNKI、维普数据库、万方数据库中有关胃肠积热证的研究文献,参考《伤寒论》阳明病热证的相关理论,按照胃肠积热证的含义、病因病机、临床表现、辨证诊断、相关性疾病5个方面汇总探析其内涵。[结果]胃肠积热证的含义是因无形热邪或有形热结而致的胃肠阳热偏盛的状态;其病因病机或是太阳病、少阳病失治误治转属阳明胃肠而致,或是饮食情志失调、素体胃肠热盛、外邪直中胃肠而致阳明胃肠本证;其临床表现以恶热、易汗出、口臭、口渴喜凉饮、大便干结、小便黄、舌红、苔黄厚、脉滑数等为主;其目前尚无客观、统一的辨证诊断标准;其与消化系统、呼吸系统、循环系统、内分泌系统和神经系统疾病的发生都有着重要的关系。[结论]胃肠积热证是一种阳热偏盛的状态,其形成或源于阳明胃肠之本,或因太阳病、少阳病失治误治而致胃肠阳热偏盛,临床表现以里热实证为主并与临床多系统疾病的发生都有着重要的关系。目前胃肠积热证尚无明确的诊断标准,因此,对胃肠积热证进一步深入研究,尤其是对胃肠积热证诊断标准的研究,对提高临床多种疾病诊疗效果和促进中医科研的发展具有重要的意义。

Objective]To clarify the concept of the Gastrointestinal Heat Retention Syndrome ,and contribute to the development of clinical diagnosis and treatment for Gastrointestinal Heat Retention Syndrome-related diseases.[Method] We have checked the researches about Gastrointestinal Heat Retention Syndrome in CNKI, VIP and Wan Fang digital database ,and referred about relevance theory of Yang Ming Heat Syndrome in Treatise on Febrile Diseases, the research is col ected and classified to five aspects on implication, etiology and pathogenesis, clinical manifestation, differential diagnosis and related diseases. [Result]The implication of Gastrointestinal Heat Retention Syndrome is because of the invisible heat evil or tangible hot junction due to gastrointestinal excess heat state; It＆#39;s etiology and pathogenesis are Tai Yang disease and Shao Yang disease mistreatments or gastrointestinal tract intrinsic heat result from improper diet and emotional maladjustment, congenital gastrointestinal excess heat, suffering exogenous evil directly; Its main clinical manifestation includes aversion to heat, easy to sweat, halitosis, thirsty to drink cold, constipation, urine yel ow, red tongue, yel ow thick tongue coating and slippery rapid pulse; It has no both objective and unitive diagnosis standard so far. It has a significant relationship with the occurrence of digestive system, respiratory system, circulatory system, endocrine system and neurological system diseases. [Conclusion] Gastrointestinal Heat Retention Syndrome means gastrointestine keeps excess heat state, it results from gastrointestinal excess heat or Tai Yang disease and Shao Yang disease mistreatment, its main clinical manifestation is interior heat syndrome, it has a significant relationship with many system diseases ,but they have different views on the diagnosis standard. Therefore, further research on the Gastrointestinal Heat Retention Syndrome, especial y on its diagnosis standard meaningful to the development of clinical diagnosis and treatment for gastrointestinal heat retention syndrome-related diseases.
