DOG1在常见涎腺肿瘤中的表达及诊断价值

摘要 目的探讨DOG1在常见涎腺肿瘤中的表达及其诊断价值。方法应用免疫组织化学En Vision二步法检测DOG1在20例正常涎腺组织及154例14种常见涎腺肿瘤中的表达情况。结果 17例腺泡细胞癌均呈DOG1阳性,其中14例(82.4%)呈弥漫阳性;85.7%(6/7)的基底细胞腺癌见DOG1染色,其中3例(42.9%)呈弥漫阳性;42.9%为(3/7)多形性低度恶性腺癌、37.5%为(6/16)基底细胞腺瘤、30%为(6/20)腺样囊性癌、25%(5/20)为多形性腺瘤、21.1%(4/19)为黏液表皮样癌呈DOG1部分阳性表达,8例为嗜酸细胞瘤/癌、5例为非特异性腺癌、3例为上皮-肌上皮癌、12例为肌上皮瘤/癌、20例Warthin瘤均DOG1阴性。结论 DOG1可以与消化PAS联合应用于腺泡细胞癌诊断及鉴别诊断;基底细胞腺癌中DOG1表达明显高于基底细胞腺瘤(P=0.0162),DOG1可能可以用于基底细胞腺瘤与基底细胞腺癌的鉴别。 Objective To investigate the expression of DOG1 in common salivary gland tumors and its diagnostic value.Methods Immunohistochemistry by En Vision Doublestain Kit was used to detect the expression of DOG1 in 20 normal salivary tissues and 154 common salivary gland tumors(a total of 14 types). Results Seventeen patients with acinic cell carcinoma were all DOG1-positive, 82.4%(14/17) were positively diffuse; DOG1 staining was noted in 85.7%(6/7)with basal cell adenocarcinoma, 3 of whom(42.9%) were positively diffuse; 42.9%(3/7) with polymorphous low-grade adenocarcinoma, 37.5%(6/16) with basal cell adenoma, 30%(6/20) with adenoid cystic carcinoma and 25%(5/20) with pleomorphic adenoma and 21.1%(4/19) with mucoepidermoid carcinoma showed partial DOG1-positive expression; 8with oncocytoma, 5 with adenocarcinoma not otherwise specified, 3 with epithelial-myoepithelial carcinoma, 12 with myoepithelioma or myoepithelial carcinoma, and 20 with Warthin's tumor were DOG1-negative. Conclusion Use of DOG1 in combination with digestive PAS can be applied in diagnosis and differential diagnosis of acinic cell carcinoma.Expression of DOG1 is markedly higher in basal cell adenocarcinoma than in basal cell adenoma(P=0.0162). DOG1 is likely to be used in differentiation between basal cell adenoma and basal cell adenocarcinoma.
