NSE和S-100蛋白在判断新生儿脑损伤及预后的价值 被引量:4

摘要 目的探究在脑损伤的新生儿中行神经特异性烯醇化酶(NSE)和S-100蛋白检测对患儿损伤程度及其预后情况的研究价值。方法选取2009年3月至2012年1月在成阳市中心医院进行治疗的新生儿脑损伤患者73例为异常组,并选取同期出生的新生儿160例为正常组,对比两组新生儿临床资料的差异性。结果异常组中静脉血的S-100蛋白和NSE高于正常组(F值分别为75.152、694.542,均P<0.05);脑脊液中S-100蛋白和NSE也高于正常组,且差异有统计学意义(F值分别为256.914、93.381、均P<0.05)。异常组脑脊液中S-100蛋白和NSE数值随预后变差而升高,差异存在统计学意义(F值分别为89.581、24.262,均P<0.05);静脉血数值变化趋势无意义。结论脑脊液、血清中NSE及S-100蛋白可对缺氧缺血性脑病(HIE)的早期判断有较大指导意义,可通过NSE和S-100蛋白水平的变化来推测患儿的远期预后。 Objective To explore the value of NSE and S-100 in judging the injury severity and prognosis of neonates.Methods From March 2009 to January 2012 73 cases with neonatal brain injury treated in Central Hospital of Xianyang were seleted in abnormal group, and another 160 cases of normal neonates in the hospital were selected at the same period in normal group.The clinical data of two groups were compared.Results In the abnormal group the contents of S-100 and NSE in venous blood were higher than in the normal group ( F value was 75.152 and 694.542, respectively, both P〈0.05).They were also higher in cerebrospinal fluid in the abnormal group, and the differences were significant (F value was 256.914 and 93.381, respectively, both P〈0.05).In the abnormal group S-100 protein and NSE values in cerebrospinal fluid increased with poor prognosis and and there was statistical signficance (F value was 89.581 and 24.262, repectively, both P〈0.05) .Change trend of venous blood was meaningless.Conclusion NSE and S-100 in cerebrospinal fluid and serum have great value in judging the early situation of hypoxic ischemic encephalopathy ( HIE) .The changes of NSE and S-100 can predict the long-term prognosis of neonates with HIE.
