阿立哌唑治疗精神分裂症的血药浓度与临床效应关系研究

Research on the plasma concentration and clinical effect relationship aripiprazole treatment of schizophrenia

目的研究阿立哌唑治疗精神分裂症的血药浓度与临床疗效之间的相关性,为临床治疗提供参考。方法选取2011年1月至2013年1月于本院就诊的72例精神分裂症患者,非固定剂量阿立哌唑治疗8周,应用反相高效液相色谱法测定患者血药浓度,采取阳性与阴性症状量表和临床总体印象以及病情严重程度量表评定疗效,分析血药浓度与临床疗效的相关性。结果治疗第2周末阳性和阴性症状量表(PANSS)总分与阳性症状(SAPS)、一般病理症状与基线数据相比均显著降低,第4、8周末各指标均显著降低,血药浓度与药物剂量呈显著正相关,第2、4、8周末血药浓度与PANSS减分率呈显著正相关。截至第8周末,72例患者中治疗有效51例,有效率为70.83%,但有效组患者的血药浓度在第4、8周末均较无效者显著升高。结论阿立哌唑治疗精神分裂症的血药浓度与临床疗效存在一定相关性。

Objective To study between aripiprazole in the treatment of schizophrenia serum concentration correlated with clinical efficacy in order to provide a reference for clinical treatment. Method Selected 72 schizophrenic patients from January 2011 to January 2013 with non-ifxed-dose aripiprazole treatment for 8 weeks, used RP-HPLC to measure plasma concentration, the application of positive and negative syndrome scale and the clinical global impression-severity scale to assess the clinical condition of the correlation between plasma concentration and clinical efifcacy analysis. Result The second weekend PANSS total score and SAPS, general pathology symptoms compared with baseline data were signiifcantly reduced, the fourth weekend with the ifrst 8 weeks of the indicators are significantly reduced. The fourth and eighth weekend plasma concentration was signiifcantly and positively correlated with the dose, plasma concentrations of 2, 4, 8 weekend PANSS reduced rate with a significant positive correlation. As of the eighth weeked, 72 patients in an effective 51 cases, the effective rate was 70.83%, but the effective blood concentration of the group in the ifrst weekend was higher than 4, 8 ineffective signiifcantly. Conclusion There is a certain correlation between aripiprazole concentrations and clinical effect.
