摘要 目的研究ALI/ARDS患者呼出气冷凝液(EBC)和血清中降钙素原(PCT)的变化规律,探讨PCT在ALI/ARDS诊断、病情评估、预后判断中的临床意义。方法选择行机械通气的ALI/ARDS患者33例;同时选择20例健康体检者为健康对照组。观察ALI/ARDS患者与健康对照组之间血清和EBC中PCT的差异,分析临床指标和各种指标之间的相关性。结果 1ALI/ARDS组EBC和血清中PCT高于健康对照组。2ARDS组患者第七天EBC和血清中PCT中PCT高于ALI组。3存活组患者治疗后EBC和血清中PCT水平下降,病死组患者EBC和血清中PCT持续升高。4EBC和血清中PCT与Pa O2/Fi O2、APACHEⅡ有相关性。结论 ALI/ARDS患者EBC和血清中PCT的检测,有助于ALI/ARDS的诊断、病情评估和判断预后。 Objective To detect the change of procalcitonin (PCT) in exhaled breath condensate (EBC) and serum of ALI/ARDS patients and to explore its clinical significance in diagnosis, illness evaluation and prognosis. Methods The study selected 33 mechanical ventilation patients with ALI/ARDS as the observation group and 20 healthy people as the control group. The difference of PCT in EBC and serum was compared between the two groups, and the correlation among the clinical indicators was analyzed. Results ① The level of PCT in EBC and serum was higher in the observation group than in the control group. ② The level of PCT was higher in the ARDS group than in the ALI group on the seventh day. ③ The level of PCT decreased in the survival group after treatment, but it increased in the death group. ④ The level of PCT in EBC and serum was associated with PaO2/FiO2 and APACHE II. Conclusion The test of PCT in EBC and serum of ALI/ARDS patients can contribute to their diagnosis, condition evaluation and prognosis.
