组织多普勒成像技术对无左心室重构青年原发性高血压患者左室舒张功能的评价 被引量:8

Evaluation of tissue Doppler imaging on left ventricular diastolic function of non-left ventricular remodeling youth sufferers with essential hypertension

摘要 目的应用组织多普勒成像(TDI)技术评价无左心室重构的青年原发性高血压(essentialhypertension,EH)患者左室舒张功能。方法选择两组对象,所有入选者年龄均小于50岁。1高血压组66例,均为无左心室重构的原发性高血压患者(即左室质量指数LVMI男性<125 g/m2、女性<120 g/m2,相对室壁厚度RWT<0.45)。2正常对照组为40例年龄、性别、体表面积与之相匹配的健康人,同期进行超声心动图检查。比较组织多普勒参数与常规超声参数评估左室舒张功能的价值。结果高血压组的左心室质量指数(LVMI)和左心室相对室壁厚度(RWT)均高于正常对照组(P<0.05),左房前后径、面积、二尖瓣E峰速度与二尖瓣环舒张早期峰值速度比值(E/Em)和左心室充盈压指标(LVFP)均显著高于对照组(P<0.01),E/A在两组差异无统计学意义(P=0.235),Em显著低于对照组(P<0.01)。高血压组E/Em与左房前后径及面积有相关关系,而E/A与二者无相关性。结论组织多普勒参数(Em,E/Em)是评价原发性高血压患者早期左室舒张功能的敏感指标。 Objective To evaluate left ventricular diastolic function of non-left ventricular remodeling youth suffer- ers with essential hypertension by means of tissue Doppler imaging technology.Methods Two groups of objects were selected and all were below 50 years old.66 cases in the hypertension group all were non-left ventricular remodeling youth sufferers with essential hypertension(LVMI : male 〈 125 g/m2, female 〈 120 g/m2 ; RWT 〈 0.45), and 40 cases in the normal control group were healthy people with matched age, sex, and body surface area who were given ultrason- ic cardiogram check.Comparison was made among tissue Doppler parameters and conventional ultrasound parameters to evaluate the values of left ventricular diastolic function.Results LVMI and RWT of the hypertension group were higher than those of the normal control group(P〈0.05).The hypertension group were remarkably higher than the nor- mal control group in terms of left atrial diameter, area, E/Era, and LVFP(P〈 0.01).E/A showed no difference of sta- tistical significance(P=0.235),and Em was clearly lower than that of the the normal control group(P〈0.01).E/Em was related to left atrial diameter and area while E/A was not related to the two factors.Conclusion Tissue Doppler parameters (Em, E/Em)are sensitive indexes to evaluate early left ventricular diastolic function of sufferers with essen- tial hypertension.
