扩张型心肌病中心脏扩大的比例与预后的关系

The Relationship between Enlarged Heart Dilated Cardiomyopathy in Proportion and Prognosis

目的:研究扩张型心肌病中左右心室的增大与预后的关系。方法:对2011年2月-2013年12月在我院治疗的80例扩张型心肌病患者进行心脏超声心动图、动态心电图检查,通过对患者心功能评级和房室大小情况来判断心脏扩张与扩张型心肌病的关系,将心功能Ⅲ级以上和Ⅲ级以下的患者进行房室大小的比较,寻求房室大小与扩张型心肌病之间的关系;将80组病例按死亡组和存活组划分,比较两组病例中的心脏大小比例。结果:扩张型心肌病的病例中左心室大的患者居多,占97.50%;心功能Ⅲ级以上和Ⅲ级以下的对比中,心功能Ⅲ级以上的患者左室有较为明显的心脏扩大趋势(P<0.001);在存活组和死亡组的对比中发现,死亡组的心脏大小比例要明显高于存活组。结论:DCM患者确诊的一个最显著特征为心脏大小比例的变化,并且根据心功能评价结果可以判定,心脏大小是DCM患者心律失常、心力衰竭、猝死的根源原因,因此有效的干预手段来帮助DCM患者控制心脏扩张,尤其是左心室扩张具有临床意义。

Objective：To increase the relationship with the prognosis of right ventricle of dilated cardiomyopathy.Methods：From 2011 February to 2013 December in 80 patients with dilated cardiomyopathy treated in our hospital were cardiac ultrasound Heartbeat graph,dynamic electrocardiogram,the heart function of patients with rating and atrioventricular size to judge the relationship between cardiac dilatation and dilated cardiomyopathy,the heart function grade and grade the following patients for comparison atrioventricular size,to seek the relation between the atrioventricular size and dilated cardiomyopathy;80 cases in death group and survival group,the two groups were compared in proportion to the size of the heart.Results：The left heart dilated cardiomyopathy cases is in the majority,accounting for 97.50%;comparative study of cardiac function of gradeⅢandⅢ below grade,heart function of left ventricle in patients with gradeⅢ above have obvious cardiac enlargement trend（P〈0.001）;found in contrast to survival group and death group,death group of heart size ratio is much higher than the survival group.Conclusion：The changes of one of the most significant features of DCM patients diagnosed as cardiac size scale,and according to the cardiac function can be determined by the results,heart size is the root cause of patients with arrhythmia,heart failure,sudden death of DCM,so the effective interventions to help patients with DCM control dilatation of the heart,especially the left ventricular dilatation with clinical significance.
