黄芪三七复方对CKD3期患者肾功能及其生活质量的影响

Assessment of Astragalus and Radix Notoginseng compound on quality of life and cost-utility of chronic kidney disease 3 stage

目的 比较复方黄芪三七结合西医基础治疗与单纯西医治疗对改善CKD进展、患者生活质量及成本-效益之间的差异.方法 选录2011年1月～2013年10月川北医学院第二临床医学院肾内科确诊为慢性肾脏病（CKD3期）病人60例,全部患者按就诊号随机分为治疗组和对照组各30例.治疗组30例,在常规西医治疗的基础上加服黄芪三七复方;对照组30例,常规西医治疗的基础上加服α-酮酸.观察周期为16周.分别于治疗前及治疗后对患者临床症状进行询问、观察并评价;比较两组肾功能、生活质量、成本-效益及不良事件发生情况作用.结果 治疗16周后两组肾功能、生活质量等均有所改善.治疗组在总有效率、总生存质量、生理、心理健康、生理职能、活力、情感及疼痛等方面的疗效优于对照组,差异均有统计学意义（P＜0.05）;在成本-效益方面也优于对照组,差异有统计学意义（P＜0.05）.两组患者均未出现严重不良事件.结论 黄芪三七复方结合西医基础治疗能明显提高CKD3期病人总有效率、改善生活质量、节约医疗成本,且安全性好.

Objective To compare the difference about improvement the progress of CKD patients and its quality of life and cost-utility between the therapeutic schedule about Western medicine combine with Astragalus and Radix Notoginseng Compound or use alone.Methods 60 patients with chronic kidney disease（CKD3period）were randomly divided into two groups：therapy group（combine traditional Chinese and western medicine,n=30）and control group（to addα-keto acid on the basis of conventional conventional western medicine treatment,n=30）.The clinical symptoms of patients before and after treatment and side-effects were observed.Results Renal function and the quality of life of two groups improved 16 weeks after treatment.The total effective rate,the total effect of the quality of life,physical,mental health,physical function,vitality,emotional and pain in the treatment group were better than that of the control group（P0.05）.The cost-utility of the treatment group was also better than that of the control group（P0.05）.There were no serious adverse events in the two groups.Conclusion Astragalus and Radix Notoginseng Compound combine with Western medicine can significantly improve the overall efficiency,improve the quality of life of patients,save medical costs and safety.
