超声充盈检查定性诊断胃良恶性溃疡性病变76例分析

Ultrasonic filling examination for diagnosis of benign or malignant gastric lesions: 76 cases analysis

目的探讨口服超声造影剂联合彩色多普勒超声对胃溃疡病变的定性诊断价值。方法选取两家医院胃超声充盈检查经胃镜活检、手术病理及随访确诊的胃良恶性溃疡76例,回顾分析其超声声像图特点。结果良、恶性溃疡在胃壁隆起增厚最大直径及厚度、胃壁黏膜下层的连续性、溃疡的形态及回声、浆膜层完整性方面存在差异,并具有统计学意义。结论胃超声充盈检查有助于定性诊断胃溃疡,具有重要的临床应用价值。

Objective To investigate the value of an integrated diagnostic method which combines oral ultrasound contrast a- gent and color Doppler ultrasonographic examination in differential diagnosis of benign and malignant gastric ulcer. Methods The sonographic features of 76 cases from two hospitals were retrospectively analyzed. The patients were definitely diagnosed as benign or malignant gastric ulcer by using gastric biopsies with endoscopy, surgical pathology and follow-up. Results There were significant differences in certain respects such as maximum diameter of thickening and thickness of the stomach bulge, continuity of the stomach submucosa, ulcers form and echo, and serosal integrity between benign and malignant ulcers. Conclusion Ultrasonography has a great clinical value in qualitative diagnosis of benign or malignant gastric ulcers.
