彩色多普勒超声在子宫切口妊娠诊断与治疗中的应用

Application of color Doppler ultrasound in diagnosis and treatment of uterine incision pregnancy

目的探讨彩色多普勒超声在剖宫产术后子宫瘢痕妊娠(cesarean scar pregnancy,CSP)的临床诊断、治疗及随访中的应用价值。方法 CSP患者38例,利用经腹、经阴道超声多方位、多切面观察孕囊或混合包块的位置、大小形态及血流情况,孕囊或混合包块与膀胱肌壁间肌层的厚度,指导临床进行分级治疗及治疗后随诊检查,并提出随访意见。结果 38例患者总体治愈率92.21%,患者恢复良好;仅3例治疗失败,失败率7.89%。结论彩色多普勒超声对CSP的早期诊断、指导临床治疗、定期随诊具有重要价值。

Objective To explore the application value of color Doppler ultrasound in clinical diagnosis,treatment and follow- up of scar pregnancy after cesarean section. Methods Thirty-eight patients with uterine incision pregnancy after cesarean section were examined with abdominal and trans-vaginal ultrasound to observe the location, size and morphology of gestational sac or mixed mass and their blood flow as well as thickness between gestational sac or mixed mass and bladder muscle wall muscle layer in multi-directions and multi views. The information was used as a basis to guide the clinical grading treatment of disease. The patients were followed-up after treatment,and put forward the follow-up observations. Results Of 38 cases, 35 cases were successfully treated （success rate was 92. 21% ） ,and the patients recovered well. Only three cases were failed （failure rate 7. 89% ）. Conclusion Ultrasound examination has an important value in early diagnosis, guidance of clinical treatment and regular fellow-up in scar pregnancy after cesarean section.
