老年患者腹腔镜直肠癌根治术与开腹手术的临床效果比较

Comparison of short-term outcomes of laparoscopic versus open surgery for rectal carcinoma in elderly patients

目的比较老年直肠癌患者腹腔镜和开腹手术的临床效果。方法分析同期行腹腔镜直肠癌根治术(腔镜组)36例和行传统开放直肠癌手术38例(开腹组)患者的临床资料,比较两组的手术时间、术中出血量、清除淋巴结数、术后肛门排气时间、术后住院时间、术后并发症等指标。结果腔镜组出血量明显少于开腹组,术后肛门排气时间和住院时间均短于开腹组,差异有统计学意义(P<0.05);两组手术时间、清除淋巴结数及术后并发症发生率差异无统计学意义(P>0.05)。结论腹腔镜直肠癌根治术应用于老年人安全有效,符合微创手术切口小、患者痛苦轻、康复快的优点及肿瘤根治原则。

Objective To compare the clinical efficiency of laparoscopic and conventional open surgery for rectal carcinoma in elderly patients. Methods The clinical records of 36 patients who underwent laparoscopic surgery（ laparoscopy group）and 38 patients who underwent conventional open surgery（ open group） were analyzed. The operative time, blood loss, numbers of lymph nodes harves- ted,passage of flatus, time of postoperative stay, and postoperative complications were compared. Results There were no significant differences in the operative time, numbers of lymph nodes harvested and postoperative complications. The blood loss was significantly less in the laparoscopy group than that in the open group. The postoperative passage of flatus and postoperative stay in the laparoscopy group were significantly shorter than that in the open group. Conclusion Laparoscopic surgery for rectal cancer in the elderly patients is a safe and feasible procedure with excellent minimally invasive effect accords with radical resection principle of malignant tumor.
