糖尿病教育管理新模式的构建及初期运用效果评价

Construction of new diabetes education management model and effect evaluation on its early use

[目的]探讨糖尿病教育管理新模式应用于2型糖尿病病人的效果。[方法]对100例2型糖尿病病人实施基于出院计划服务理念的糖尿病教育管理新模式,评价干预前和运用健康教育管理3个月、6个月后的糖尿病知识水平、自我管理水平和糖化血红蛋白的变化。[结果]干预后病人糖尿病知识评分、用药依从性及足部护理评分明显升高(P<0.01),糖化血红蛋白水平明显降低(P<0.01)。[结论]基于出院计划服务理念的糖尿病教育管理新模式可提高2型糖尿病病人的糖尿病知识水平和自我管理水平,提高糖化血红蛋白达标率。

Obj ective：To probe into the effect of new diabetes education management model applied for the patients with type 2 diabetes.Meth-ods：A total of 100 cases of patients with type 2 diabetes received new dia-betes education management model based on the concept of discharge planning services,then to evaluate the diabetes knowledge,self manage-ment level and changes of glucose metabolism before intervention and af-ter 3 months and 6 months of health education management.Results：Af-ter intervention,the diabetes knowledge score,medication compliance score and foot care score of patients increased significantly（P〈0.01）,and the glycosylated hemoglobin level significantly decreased（P〈0.01）.Con-clusion：New diabetes education management model based on discharge planning services concept could improve diabetes knowledge mastering and self management level of type 2 diabetes patients and improve com-pliance rate of glycated hemoglobin.
