2013年江西省基层医疗卫生机构基本诊疗路径实施情况调查

Basic clinical pathway in basic medical institutions of Jiangxi Province in 2013

目的:对江西省11个设区市的57种常见疾病基本诊疗路径试点情况进行定量分析,为在该省建立和完善基层医疗机构诊疗路径管理制度提供依据。方法:按照分层、整群、多阶段抽样方法,随机抽取江西省11个设区市的767所乡镇卫生院进行调查,并利用SPSS21.0进行分析。结果:江西省11个设区市的基层医疗机构试点后平均治疗费用比试点前减少了19.92%(t=34.930,P=0.000),平均治疗天数比试点前缩短了14.97%(t=4.928,P=0.000)。结论:基本诊疗路径的实施可以减少治疗费用,缩短治疗时间,说明路径的实施可以减轻患者负担,符合新医改的初衷和老百姓的根本利益,基本诊疗路径管理应该以制度的形式确立下来,并不断进行修改完善。

Objective To make quantitative analysis on the pilot work of the basic clinical pathway of 57 common diseases in basic medical institutions of 11 cities in Jiangxi Province,in order to provide the basis for setting up and improving the clinical pathway management system in the basic medical institutions of Jiangxi Province.Methods Stratified,cluster,multistage sampling method was adopted to investigate 767 basic medical institutions selected from 11 cities in Jiangxi Province and a statistical analysis was made by SPSS21.0.Results The average treatment cost was reduced by19.92%（t=34.930,P=0.000）after the basic clinical pathway was implemented in basic medical institutions of 11 cities in Jiangxi Province,and the average treatment duration was shortened by 14.97%（t=4.928,P=0.000）.Conclusions The implementation of basic clinical pathway can reduce the average treatment costs of common diseases in basic medical institutions,and can shorten the average duration of treatment,which preliminarily verifies that the basic clinical pathway can reduce the burden of the patients.Basic clinical pathway management should be established in the form of a system and modified continuously until perfection.
