2型糖尿病视网膜病变功能磁共振成像研究 被引量:1

A fMRI Study in Type 2 Diabetic Retinopathy

摘要 为了探讨在静息状态下2型糖尿病引发的视网膜病变的脑部活动情况,对21名性别、年龄、文化程度相近的2型糖尿病视网膜病变(type 2diabetes mellitus retinopathy,T2DR)患者和与之对应的16名正常志愿者(normal control,NC)在静息态下进行脑部扫描。分别用低频振幅(amplitude of low frequency fluctuation,ALFF)和局部一致性(regional homogeneity,ReHo)的脑功能磁共振成像方法 (functional magnetic resonance imaging,fMRI)处理所得的fMRI数据,对ALFF和ReHo两种方法所得的数据分别进行双样本T-检验,两种方法结合起来比较正常人和病人的脑功能活动之间的差异,并对结果进行分析。经过双样本T-检验之后的结果显示(与NC组相比):ALFF显著增高的脑区有右侧舌回、枕叶和楔叶(p<0.001,体素值>30),ALFF显著降低的脑区有右侧小脑后叶、小脑前叶、缘叶、海马旁回、梭状回、颞上回、顶下小叶、角回;ReHo显著增高的脑区有右侧舌回和枕叶,ReHo显著降低的脑区有左侧后扣带回、缘叶、右侧顶下小叶、颞上回、海马、枕叶。结论显示2型糖尿病视网膜病变患者静息态脑功能存在明显异常,ALFF和ReHo的联合运用可以有助于2型糖尿病视网膜病变患者的脑功能状况的研究。 To investigate the activity in the brain of type 2 diabetes mellitus retinopathy patients by the resting state,type 2 diabetes mellitus retinopathy patients (21) were compared with nondiabetic sex,age and education matched control subjects (16) using rs-fMRI.And then the method of ALFF and ReHo were used to process the fMRI data,and two-sample T-tests were performed on the two groups data,respectively.Finally,the two methods were combined to compare the difference brain activity between T2DR and NC,and the results were analyzed.According to the results by two-sample T-tests (Compared with NC group),the ALFF values of T2DR patients increased significantly in the right lingual Gyrus,occipital lobe and cuneus (p〈0.001,voxel〉30).The ALFF values decreased significantly in right cerebellum posterior lobe,cerebellum anterior lobe,limbic lobe,parahippocampa gyrus,fusiform gyrus,superior temporal gyrus,inferior parietal lobule and angular gyrus.The ReHo values increased significantly in right lingual Gyrus and occipital lobe.The ReHo values decreased significantly in left posterior cingulate,limbic lobe,right inferior parietal lobule,superior temporal gyrus,hippocampa and occipital lobe.There are obvious abnormalities in the brain function of T2DR patients by rs-fMRI.A combination with the methods of ALFF and ReHo will apply to the study of brain function status for T2DR patients.
