支气管肺炎患儿外周血调节性T淋巴细胞和辅助性T淋巴细胞17及其细胞因子水平变化

Changes of the levels of peripheral blood regulatory T lymphocyte cell,helper T lymphocyte cell and their cytokines in children with bronchial pneumonia

摘要 目的探讨支气管肺炎患儿外周血调节性T淋巴细胞(Treg)和辅助性T淋巴细胞17(Th17)及其细胞因子白细胞介素(IL)-10、转化生长因子-β(TGF-β)、IL-17水平的变化及意义。方法选取2012年2月至2013年12月在新安县人民医院接受治疗的支气管肺炎患儿41例作为观察组,并选取30例健康儿童为对照组;采用流式细胞仪检测外周静脉血全血中CD4+、CD8+、Treg和Th17水平,计算Treg/Th17、CD4+/CD8+比值;采用酶联免疫吸附测定法检测外周静脉血中IL-10、TGF-β和IL-17水平。结果观察组患儿CD4+细胞的比例显著高于对照组(P<0.05),2组小儿CD8+细胞的比例比较差异无统计学意义(P>0.05);观察组患儿CD4+/CD8+比值显著低于对照组(P<0.05)。观察组患儿外周血Treg比例显著低于对照组(P<0.05),Th17比例显著高于对照组(P<0.05)。观察组患儿血IL-10、TGF-β水平显著低于对照组(P<0.05),而IL-17水平显著高于对照组(P<0.05)。结论支气管肺炎患儿外周血存在外周血T淋巴细胞亚群失衡,尤其是Treg/Th17细胞及其细胞因子的表达失衡,可能是支气管肺炎的发病机制之一。 Objective To study the significance of the changes of the levels of peripheral blood regulatory T lymphocyte cell(Treg),helper T lymphocyte cell 17(Th17),interleukin(IL)-10,transforming growth factor-β(TGF-β) and IL-17 in children with bronchial pneumonia. Methods Forty-one children with bronchial pneumonia in the People' s Hospital of Xin' an County from February 2012 to December 2013 were selected as the observation group,and thirty healthy children were selected as the control group. The levels of peripheral blood CD4^+,CD8^+,Treg cell and Th17 cell were detected by flow cytometry in the two groups. Then the ratios of Treg /Th17 and CD4+/CD8^+were calculated. The levels of peripheral blood IL-10,TGF-βand IL-17 were detected by enzyme-linked immunosorbent assay in the two groups. Results The CD4+cell ratio in observation group was significantly higher than that in control group(P〈0. 05). There was no significant difference in CD8^+cell ratio between the two groups(P〉0. 05). The CD4+/CD8^+ratio in observation group was significantly lower than that in control group(P〈0. 05). The peripheral blood Treg cell ratio in observation group was significantly lower than that in control group(P〈0.05). The peripheral blood Th17 cell ratio in observation group was significantly higher than that in control group(P〈0. 05). The levels of blood IL-10 and TGF-β in observation group was significantly lower than those in control group( P〈0. 05). The level of blood IL-17 in observation group was significantly higher than that in control group( P〈0. 05). Conclusion The unbalance of peripheral blood T lymphocyte subsets exist in children with bronchial pneumonia,especially the unbalance of Treg /Th17 cells and their cytokines. It may be one of the pathogenesis of bronchial pneumonia.
