成人斜视患者生存质量状况及其影响因素调查分析

Status and influencing factors of quality of life in strabismus patients

目的探讨成人斜视患者生存质量状况及其影响因素。方法采用成人斜视患者生存质量量表、医院焦虑抑郁量表、简易应对方式量表和社会支持评定量表分别对304例成人斜视患者进行调查。结果成人斜视患者生存质量总分为(61.41±18.23)分,其中身心社会维度为(58.70±21.63)分,生理功能维度为(64.13±19.71)分;女性、年龄≥30岁、高中、内斜、有复视的患者生存质量较差;成人斜视患者生存质量量表总分与社会支持、积极应对得分呈正相关,相关系数分别为0.693(P<0.01)、0.160(P<0.05),与消极应对、焦虑及抑郁得分呈负相关,相关系数分别为-0.218(P<0.01)、-0.472(P<0.01)、-0.454(P<0.01);多因素回归分析显示:社会支持、焦虑、性别及有无复视是成人斜视患者生存质量主要影响因素。结论成人斜视患者生存质量总体不容乐观,且受到多种因素影响。应加强斜视患者医疗保障体系,多给予社会支持,并加强健康宣教工作,提高患者生存质量。

Objective To investigate the status of quality of life （QOL） and its influencing factors of strabismus in adults. Methods Convenience sampling was performed on all adult out-patients with strabismus in one Grade A Class 3 hospital in Chongqing from January 2012 to June 2013. Then a total of 304 obtained patients were surveyed with the adult strabismus questionnaire （AS-20）, hospital anxiety and depression scale, simplified coping style questionnaire and social support review survey. Results The score of QOL in the cohort was 61.41 ± 18.23, with the score of psychosocial dimension 58.70 ±21.63, and the score of physical function dimension 64.13 ± 19.71. Female, over 30 years old, educational level under senior middle school, and suffering from esotropia and diplopia had a worse QOL. The total score of QOL was positively correlated with the scores of social support and positive coping style, with correlation coefficients of 0.693 （ P 〈 0. 01 ） and 0. 160 （P 〈 0. 05 ）, respectively. The total score of QOL was negatively correlated with the scores of negative coping style, anxiety and depression, and the correlation coefficients were -0. 218, -0. 472 and -0. 454, respectively （all P 〈0. 01 ）. Multi-factor regression analysis demonstrated that social support, anxiety , sex, and with or without diplopia were influencing factors for patients＇ QOL. Conclusion The adult strabismus patients are commonly in poor QOL, which are affected by multiple factors. More efforts should be done to strengthen health service mode, afford sufficient social support and emphasize health education so as to improve the quality of life of them.
